静风汤联合硫必利治疗小儿抽动障碍脾虚肝旺证

摘　　要：目的:观察自拟静风汤联合硫必利治疗小儿抽动障碍脾虚肝旺证的临床疗效。方法:120例小儿抽动障碍脾虚肝旺证患儿按数字表法随机分为对照组(n=59)和研究组(n=61)。对照组给予盐酸硫必利片,研究组在对照组治疗基础上给予自拟静风汤治疗,疗程均为3个月。比较两组患者治疗后的临床疗效、治疗前后的抽动程度、中医证候评分、儿童行为变化及治疗期间的不良反应。结果:治疗后,研究组有效率为91.8%(56/61),对照组为76.27%(45/59),两组有效率比较,差异有统计学意义(P<0.05)。治疗后研究组发声性抽动、运动性抽动耶鲁综合抽动严重程度量表评分低于对照组(P<0.05);治疗后研究组中医证候(运动抽动、发声抽动、精神倦怠、烦躁易怒与纳差)评分均低于对照组(P<0.05);治疗后研究组Achenbach儿童行为量表评分低于对照组(P<0.05)。治疗期间研究组不良反应发生率低于对照组(P<0.05)。结论:自拟静风汤联合硫必利治疗小儿抽动障碍脾虚肝旺证临床疗效确切,能显著改善患儿的临床症状、中医证候评分及儿童的行为问题,且不良反应低。Objective:To observe the clinical effect of self-made Jingfeng Decoction combined with tiapride in treating tic disorder in children with syndrome of spleen deficiency and liver hyperactivity.Methods:120 children with tic disorder were randomly divided into control group(n=59)and study group(n=61).The control group was treated with tiapride hydrochloride tablets,and the study group was treated with self-made Jingfeng Decoction on the basis of the treatment of the control group for 3 months.The clini-cal effect,tic degree,TCM syndrome score,children's behavior change and adverse reactions during the treatment were compared between the two groups.Results:After treatment,the effective rate was 91.8%in the study group and 76.27%in the control group,the difference was statistically significant(P<0.05).After treatment,the score of Yale comprehensive twitch severity scale of vocal twitch and motor twitch in the study group was lower than that in the control group(P<0.05);the score of TCM syn-dromes(motor twitch,vocal twitch,mental burnout,irritability and poor appetite)in the study group was lower than that in the control group(P<0.05).After treatment,the scores of Achenbach children's behavior scale in the study group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).Conclusion:The clinical effect of self-made Jingfeng Decoction combined with tiapride in treating tic disorder in children with syndrome of spleen deficiency and liver hyperactivity is definite,which can significantly improve the clinical symp-toms,TCM syndrome changes and children's behavior problems,and the adverse reactions are low.
