中西医结合防治类风湿关节炎合并萎缩性胃炎

摘　　要：类风湿关节炎合并萎缩性胃炎患者,多以脾肾阳虚寒湿困阻为主,常兼有阴虚火炎、气滞血瘀、津亏血燥等证。中医根据舌象的变化,预测疾病的发展趋势,如出现花剥舌、苔少舌或镜面舌时,提示已有胃部损伤,可及时调整药物防治。对于类风湿关节炎因寒凝血瘀而疼痛时,予以温阳散寒、活血通络止痛药物;肢体关节重浊冷痛时,可予以祛寒除湿、通络止痛药物;有游走性关节疼痛时,可加用祛风通络之药;关节红肿热痛时,可予以清热化湿、解毒散结、消肿止痛药物。对于合并萎缩性胃炎,或仅有萎缩性胃炎相关症状及少苔患者,予以温阳益气健脾化湿兼养阴清热润燥、益肾健脾柔肝等药物,佐用少量虫类药,可明显改善患者胃脘疼痛、胀满不舒等症状。Patients with rheumatoid arthritis complicated by chronic atrophic gastritis are mostly Spleen-Kidney Yang deficiency Syndrome with Cold-Dampness stagnation,and it is sometimes companied with Yin deficiency Fire inflammation,Qi stagnation and Blood stasis as well as Jin deficiency and Blood dryness. According to the change of tongue image,the development trend of the disease can be predicted with Chinese medicine,such as the occurrence of flower peeling tongue,moss-less tongue or mirror tongue. That indicates that there is already stomach injury,and the drug control can be adjusted in time. When rheumatoid arthritis is painful due to cold coagulation and Blood stasis,Yang warming and Cold dispelling as well as Huoxue Tongluo analgesic method can be used;when the limbs and joints are heavy and cold,pain can be relieved by cold and dehumidification,Tongluo analgesia;The medicine for removing wind and collaterals can be added;when the joints are red,swollen hot and painful,the Heat-Dampness should be cleared away as well as detoxification and dissolving method should be used to relieve swelling and pain. For patients with atrophic gastritis,or only with atrophic gastritis-related symptoms and less moss,Yang warming,Qi tonifying,Spleen strengthening and Dampness resolving method should be used,which can be accompanied with Yin tonifying,Heat clearing and Dryness moistening as well as Kidney,Spleen tonifying and softening Liver Qi.,with a small amount of insect drugs. The theory can significantly improve the symptoms of epigastric pain and bloating.
