摘　　要：目的:评价中医药治疗乳腺癌癌前病变的临床疗效。方法:检索中国知网、万方数据库、维普数据库、PubMed、Cochrane全文数据库,纳入自建库始至2018年11月的中医药治疗乳腺癌癌前病变的随机对照试验,检索关键词为:"中医药治疗"AND"乳腺癌癌前病变OR巨乳症OR乳晕湿疹OR乳腺囊性增生病OR乳管内乳头状瘤OR乳腺纤维腺瘤OR乳腺腺瘤",采用Review Manger 5.3软件进行结果分析。结果:共纳入39篇随机对照试验文献,共4731例乳腺癌癌前病变患者。所纳入文献质量评分为1~7分。Meta分析结果:中医药治疗乳腺癌癌前病变,临床总体有效率[OR=0.21,95%CI(0.17,0.25),P<0.00001]、复发率[OR=4.87,95%CI(3.66,6.47),P<0.00001]、不良反应率[OR=4.75,95%CI(2.84,7.94),P<0.00001]、乳房肿胀率[MD=0.98,95%CI(0.49,1.48),P=0.00001]、乳房疼痛情况[MD=2.84,95%CI(1.38,4.30),P=0.0001]、乳房皮肤红肿情况[MD=1.77,95%CI(0.81,2.73),P=0.0003]及患者生活质量情况[OR=-18.43,95%CI(-20.39,-16.48),P<0.00001]等方面均优于对照组。结论:中医药治疗乳腺癌癌前病变疗效较好,在治愈率及总体有效率方面有一定的优势,复发率较少,安全性较高。Objective:To evaluate the clinical efficacy of traditional Chinese medicine in treating breast cancer precancerous lesions.Methods:The Chinese Knowledge Network(CNKI),Wanfang database,Weipu database,PubM ed,and Cochrane full-text databases were searched.They were included in the randomized controlled trials of Chinese medicine for breast cancer precancerous lesions since the establishment of the database until November 2018.The keywords searched were("Traditional Chinese Medicine Treatment")AND("Breast cancer precancerous lesions OR giant mastopathy OR areola eczema OR breast cystic hyperplasia OR intrapapillary papilloma OR breast fibroadenoma or breast adenoma"),review manger 5.3 software was applied for result analysis.Results:A total of 39 randomized controlled trial literatures were included,with a total of 4,731 patients with breast cancer precancerous lesions.The included literature quality scores ranged from 1 to 7.Meta analysis results show that:TCM treatment of breast cancer precancerous lesions,the overall clinical effectiveness[OR=0.21,95%CI(0.17,0.25),P<0.00001],recurrence rate[OR=4.87,95%CI(3.66,6.47),P<0.00001],adverse reaction rate[OR=4.75,95%CI(2.84,7.94),P<0.00001],breast swelling rate[MD=0.98,95%CI(0.49,1.48),P=0.00001],breast pain[MD=2.84,95%CI(1.38,4.30),P=0.0001],breast skin swelling[MD=1.77,95%CI(0.81,2.73),P=0.0003],and patient survival The quality situation[OR=-18.43,95%CI(-20.39,-16.48),P<0.00001]and other aspects were better than the control group.Conclusion:Traditional Chinese medicine has good curative effect on precancerous lesions of breast cancer.It has certain advantages in cure rate and overall effective rate,with less recurrence rate and higher safety.
