摘　　要：目的利多卡因硬膜外神经阻滞联合加巴喷丁对带状疱疹后神经痛患者疼痛、睡眠及生活质量的影响。方法将2016年1月—2017年12月就诊治疗的52例带状疱疹后神经疼痛患者作为研究对象。根据用药方案的不同分为对照组和观察组,每组26例。对照组应用加巴喷丁口服治疗,观察组在对照组基础上联合利多卡因硬膜外神经阻滞术进行治疗。对2组治疗前后的疼痛、睡眠状况、生活质量进行比较,并观察治疗后不良反应的发生情况。结果2组治疗后1、3、7、15和30 d时疼痛视觉模拟量表评分、睡眠干扰评分以及生活质量评分较治疗前均显著下降(P<0.05),其中对照组上述评分均明显高于观察组(P<0.01)。治疗后对照组不良反应总发生率明显高于观察组(P<0.01)。结论利多卡因硬膜外神经阻滞术与加巴喷丁联合应用于带状疱疹后神经疼痛的治疗,降低疼痛效果更佳,不良反应发生率低,有利于改善患者睡眠及生活质量。Objective To investigate the effects of lidocaine epidural nerve block combined with gabapentin on pain,sleep and quality of life in patients with postherpetic neuralgia(PHN).Methods A total of 52 patients with PHN treated from January 2016 to December 2017 were selected as research subjects.The patients were divided into control group(n=26)and observation group(n=26),according to different medication regimens.The control group was treated with gabapentin orally,and the observation group was treated with Lidocaine epidural nerve block on the basis of the control group.The pain,sleep status and quality of life of the two groups before and after treatment were compared,and the incidence of adverse reactions after treatment was observed.Results Visual analog scale pain score,sleep interference score and quality of life score of the two groups were decreased significantly at 1,3,7,15 and 30 d after treatment compared with those before treatment(P<0.05),while the above scores of the control group were significantly higher than those of the observation group(P<0.01).The total incidence of adverse reactions in the control group was significantly higher than that in the observation group(P<0.01).Conclusion Lidocaine epidural nerve block combined with Gabapentin can be used in the treatment of PHN,which has a better effect of reducing pain,a lower incidence of adverse reactions,and is conducive to improving the patients'sleep and quality of life.
