摘  要:目的探讨一次性尿袋不同更换频率对患者尿路感染发生率的影响。方法检索中国生物医学文献库(CBM)、中国学术期刊全文数据库(CNKI)、万方数据资源系统(WANFANG DATA)、维普中文期刊全文数据库(VIP)、Pubmed、EMbase、CIHAHL、Google Scholar、ProQuest Dissertations&Theses。收集一次性尿袋1 d更换(每日)、3 d更换(包括每周2次)、7 d更换(包括每周1次)的随机对照研究,用RevMan 5.1.0软件进行分析。结果共纳入15篇文献。其中每日更换尿袋和每3天更换尿袋的有10项研究,进行效果合并后每3天更换尿路感染的发生率较每日更换低[OR=2.22,95%CI(1.35,3.63),P=0.002];其中每3天更换与每7天更换的有11篇研究,进行效果合并后每3天更换尿袋和每7天更换尿袋的尿路感染发生率无差异[OR=0.68,95%CI(0.34,1.35),P=0.27]。结论每3天更换尿袋和每7天更换尿袋尿路感染发生率无统计学差异,且皆低于每日更换,考虑护士工作量和经济因素,更加推荐对于一次性尿袋每7天更换1次。Objective To investigate the impact of different frequency of disposable urine bags on the incidence of urinary tract infections in patients.Methods Literature searches were performed in databases such as China Biology Medicine disc(CBMdisc),China National Knowledge Infrastructure(CNKI),WANFANG DATA,China Science and Technology Journal Database(VIP),Pubmed,EMbase,CIHAHL,Google Scholar and ProQuest Dissertations&Theses.Randomized controlled studies involving the frequency of disposable urine bag changes(including daily replacement,3-day replacement,and 7-day replacement)were included in the literature and analyzed using RevMan 5.1.0 software.Results A total of 15 articles were included in this study.Combining the effects of 10 studies involving daily replacement and 3-day replacement,the incidence of urinary tract infection of 3-day replacement was lower than daily replacement[OR=2.22,95%CI(1.35,3.63),P=0.002];there is no difference in the incidence of urinary tract infections between the 3-day replacement and 7-day replacement after combining the effects of 11 studies[OR=0.68,95%CI(0.34,1.35),P=0.27].Conclusion The incidence of urinary tract infections per day is higher than changing urinary bags every 3 days and urinary bags every 7 days,but there is no statistics difference on the incidence of urinary tract infection between changing urinary bags every 3 days and urinary bags every 7 days.Considering the workload of nurses and economic factors,this study recommends replacing disposable urine bags every 7 days.
