摘 要:目的调查并了解长沙地区Rh D阴性献血者血清学表型分布,为建立Rh D阴性稀有血型库提供依据。方法对长沙血液中心2016年1月-2018年5月的献血者标本375 710人份,使用微板法进行Rh D抗原筛查;进一步采用试管抗人球蛋白法对初筛阴性者进行确认;确认阴性者C、c、E、e表型鉴定则采用盐水试管法。结果 375 710名献血者中初筛Rh D阴性1 483例,其中D变异型47例,确认Rh D阴性1 436例(96.83%)。确认阴性中A型482例(33.57%),B型305例(21.24%),O型534例(37.19%),AB型115例(8.01%);表型鉴定结果为ccdee 759例(52.86%),Ccdee 520例(36.21%),CCdee 78例(5.43%),ccdEe 50例(3.48%),CcdEe 26例(1.81%),ccdEE 2例(0.14%),CCdEe 1例(0.07%)。结论对Rh D阴性献血者进行血清学表型鉴定,并建立Rh D阴性资料库,对保障Rh D阴性患者临床用血安全及时有效具有重要意义。Objective To investigate and understand the serological phenotype distribution of Rh D-negative blood donors in Changsha, and to provide a basis for the establishment of a Rh D-negative rare blood group bank. Methods A total of 375710 blood donor specimens from January 2016 to May 2018 at Changsha Blood Center were screened for Rh D antigen using the microplate method;further in vitro anti-human globulin method was used to confirm negative screening;confirmation The C, c, E, and e phenotypes of the negative patients were determined by the saline test tube method. Results In 375,710 blood donors, 1 483 Rh D-negative patients were screened, including 47 D variants, and 1 436 Rh D-negative patients(96.83%) were confirmed. Among the confirmed negative, 482 cases(33.57%) of type A, 305 cases(21.24%) of type B, 534 cases(37.19%) of type O, and 115 cases(8.01%) of type AB;759 cases(52.86%) of ccdee were identified, 520 cases of Ccdee(36.21%), 78 cases of CCdee(5.43%), 50 cases of ccdEe(3.48%), 26 cases of CcdEe(1.81%), 2 cases of ccdEE(0.14%), 1 case of CCdEe(0.07%). Conclusion Serological phenotypic identification of Rh D-negative blood donors and the establishment of a Rh D-negative database are of great significance for ensuring the safe and timely use of blood in Rh Dnegative patients.
