痛风方内服配合四黄散外敷治疗急性痛风性关节炎的经验研究

摘　　要：目的:观察王海东教授多年使用痛风方内服配合四黄散外敷治疗急性痛风性关节炎(AGA)的临床疗效及部分作用机制。方法:选取AGA患者80例,随机分为对照组、观察组,对照组采用口服双氯芬酸钠缓释胶囊治疗,观察组采用内服痛风方配合四黄膏外敷治疗。比较两组患者的临床疗效、中医证候积分、血清生化指标等。结果:治疗后观察组的总有效率92.31%(36/39),明显高于对照组79.49%(31/39)(P<0.05)。两组治疗后关节疼痛、肿胀、发红、功能受限积分降低(P<0.05或P<0.01);观察组治疗后关节疼痛、肿胀、发红、功能受限积分低于对照组(P<0.05或P<0.01)。两组治疗后外周血WBC、UA、CRP、ESR水平显著下降(P<0.05或P>0.01);观察组治疗后WBC、UA、CRP、ESR水平低于对照组(P<0.05或P>0.01)。结论:痛风方内服配合四黄散外敷治疗AGA临床疗效确切,可显著改善关节症状,降低血清WBC、UA、CRP、ESR水平,调节机体内炎性反应是其作用机制之一。Objective:To observe the empirical research of internal administration of Tongfeng Formula(痛风方)combined with external application of Sihuang Powder(四黄散)in the treatment of acute gouty arthritis.Methods:80 patients with acute gout arthritis attack were randomly divided into the control group and the treatment group.Patients in the control group were treated with Diclofenac sodium sustained release capsule administration;while patients in the treatment group were treated with internal administration of Tongfeng Formula combined with external application of Sihuang Powder.With reference to Guiding Principles of Clinical Research on New Medicines of Traditional Chinese Medicine,the curative effect was assessed.Results:The curative effects of the two groups were compared(the total effectiveness of the treatment group was 92.31%,while that of the control group was 79.49%)(P<0.05).Compared with the control group,the white blood cell(WBC),C reactive protein(CRP),erythrocyte sedimentation(ESR)and serum uric acid(Ua)were significantly improved in the observation group(P<0.05).Conclusion:The clinical effect of internal administration of Tongfeng Formula combined with external application of Sihuang Powder was accurate.It can significantly relieve joint symptoms,decrease the t white blood cell(WBC),C reactive protein(CRP),erythrocyte sedimentation(ESR)andserum uric acid(Ua)and regulate the inflammatory response in the body.
