基于移动端平台的延续护理在老年衰弱患者中的效果观察

摘　　要：目的探讨基于移动端平台的老年衰弱患者延续护理的应用效果。方法选取老年科出院的老年衰弱患者60例,随机分为观察组和对照组各30例。对照组实施常规出院指导及随访,观察组在对照组的基础上开展基于移动端平台的延续护理。观察2组出院当天、出院后1个月后和出院后3个月后,2组患者的Barthel指数、Braden压疮风险评估量表、误吸风险评价结果。结果出院1、3个月后,2组Barthel指数、Braden压疮风险评估量表评分均高于出院当天,且观察组高于对照组,差异有统计学意义(P<0.05)。出院1、3个月后,2组误吸风险评分均低于出院当天,且观察组低于对照组,差异有统计学意义(P<0.05)。结论对老年衰弱患者,应用移动端平台开展延续护理,可提高患者日常生活活动能力,降低护理风险。Objective To investigate the effect of extended nursing model based on the mobile terminal platform in senile frailty patients.Methods Totally 60 senile patients in a condition of frailty after discharge from hospital were randomly divided into the observation group and the control group,with 30 cases in each group.All patients received routine health guidance and follow-up,and a extended nursing model based on the mobile terminal platform was used in the observation group.All patients were investigated by the Barthel Index,Braden Scale for pressure ulcer risk assessment and risk assessment of aspiration at day of discharge,1 month and 3 months after discharge form hospital.Results The score of Barthel Index and Braden Scale for pressure ulcer risk assessment were increased at 1 month and 3 months after discharge form hospital,and were higher in the observation than those in the control group(P<0.05).The score for risk assessment of aspiration was decreased at 1 month and 3 months after discharge form hospital,and was lower in the observation than that in the control group(P<0.05).Conclusion The extended nursing model based on the mobile terminal platform is potentially effective to improve the senile frailty patients'activity of daily living and reduce risk of nursing adverse events.
