党中勤从脾论治肝硬变腹水经验

摘　　要：党中勤教授认为肝硬变腹水是由于肝病日久,肝病传脾或者脾虚肝乘,导致脾虚运化失职,水湿内停,疏泄不利,体内水邪泛滥,是本虚标实之证,本虚为脾,标实为水。在肝硬变腹水的治疗中,党老师非常注重实脾,着力于补益脾气,使脾运化水湿功能恢复正常,气机调畅,水湿得化。在培补中宫脾土的同时,党老师也非常注重水肿的消除,临床治疗中加入四苓散以健脾渗湿、利水消肿,以治水湿内停、小便不利、水肿泄泻,且不妄用攻下逐水药物。党老师继承前人理论,结合自己的临床经验,以健脾益气、活血利水为大法,总结出临证用药经验方黄芪泽苓汤,在临床应用中取得了满意疗效,能够消除腹水,提高血浆白蛋白。Professor Dang Zhong-qin thinks that liver cirrhosis ascites is due to liver disease for a long time.Liver disease spreads to the spleen or spleen deficiency influences the liver,which leads to the spleen deficiency and failure to function,water retention,unsmooth release stagnated energy,excessive water overflows.This is the syndrome of root deficiency and branch excess.The root deficiency lies in the spleen,and the branch excess involves in water.In terms of liver cirrhosis ascites,Professor Dang focuses on nourishing spleen qi to recover spleen's function of transportation and transformation of water,regulate qi movement.Besides nourishing the spleen,Professor Dang not only pays attention to removing edema.In clinical practice,Powder of Four Drugs for Diuresis is often used to invigorate spleen and remove dampness,promote diuresis to reduce edema,but also is very cautious in using drugs for elimination of water retention by catharsis.Professor Dang inherits the predecessor theory,unifies own clinical experience,takes invigorating the spleen and benefiting qi,promoting blood circulation and dieresis as the principal method,summarizes the clinical medication experience formula:Astragalus and Rhizoma Alismatis and Poria Decoction,which can eliminate ascites and enhances the plasma albumin,and has obtained the satisfactory curative effect in the clinical application.
