身痛逐瘀汤加味联合针灸疗法治疗腰椎间盘突出症的临床疗效

摘　　要：目的:观察身痛逐瘀汤加味联合针灸疗法治疗腰椎间盘突出症(Lumbar disc herniation,LDH)的临床疗效。方法:选取100例腰椎间盘突出症患者为研究对象,按照就诊先后顺序将其分为治疗组和对照组,每组各50例。对照组采用身痛逐瘀汤加味治疗,治疗组在对照组基础上增加针刺疗法。对比分析两组的临床疗效、治疗前后疼痛症状、腰椎功能改善情况、中医症状积分、血流变指标变化及并发症情况。结果:治疗组临床疗效高于对照组,差异有统计学意义(P<0.05);治疗后视觉疼痛模拟评分(Visual Analogue Scale,VAS)均较治疗前降低,但治疗组低于对照组(P<0.05);治疗后腰椎功能评估量表(Japanese Orthopaedic Association,JOA)均较治疗前升高,但治疗组高于对照组(P<0.05);治疗后Oswestry功能障碍指数问卷表(ODI)均较治疗前降低,但治疗组低于对照组(P<0.05);两组治疗后中医症状积分均显著低于治疗前,且治疗组治疗后显著低于对照组(P<0.05);与治疗前相比,血液流变学各项指标均明显降低,但治疗组优于对照组(P<0.05);两组均未出现明显的不良反应。结论:身痛逐瘀汤加味联合针灸疗法治疗腰椎间盘突出症疗效显著,有效改善患者疼痛症状,改善患者腰椎功能,提高生活质量,安全可靠,值得临床借鉴。Objective:To observe the clinical efficacy of the Shentong Zhuyu decoction(身痛逐瘀汤)plus acupuncture on lumbar disc herniation.Methods:100 patients with lumbar disc herniation were selected as the study subjects.They were divided into the treatment group and control group according to the order of treatment,50 cases in each.The control group was treated with the Shentong Zhuyu decoction,and the treatment group was treated by acupuncture more.The clinical efficacy,pain symptoms before and after treatment,improvement of lumbar function,scores of TCM symptoms,changes of hemorheological parameters and complications were compared.Results:The clinical efficacy in the treatment group was higher than that in the control group,the difference was statistically significant(P<0.05).The VAS was lower than that before treatment,but that in the treatment group was lower than the control group(P<0.05).The JOA was higher than that before treatment,but that in the treatment group was higher than the control group(P<0.05).The ODI was lower than that before treatment,but that in the treatment group The control scores of TCM were significantly lower than those before treatment,and that in the treatment group was significantly lower than the control group(P<0.05).Hemorheology was compared with before treatment.All indexes were significantly lower,but those in the treatment group was superior to the control group(P<0.05);no significant adverse reactions occurred in either group.Conclusion:The treatment of lumbar intervertebral disc herniation with the Shentong Zhuyu decoction plus acupuncture is effective,improve the pain symptoms,improve the lumbar function of the patient,improve the quality of life,safe and reliable,and it is worthy of clinical reference.
