非共面与共面VMAT技术应用于鼻腔NK/T细胞淋巴瘤调强放疗的剂量学差异  被引量:4

摘  要:目的:探讨非共面容积旋转调强放疗(volumetric modulated arc therapy,VMAT)技术相对于共面VMAT技术在鼻腔NK/T细胞淋巴瘤调强放疗中是否存在优势。方法:选择10例鼻腔NK/T细胞淋巴瘤患者,分别在Raystation 4.7.5计划系统上设计非共面VMAT计划和共面VMAT计划,评估2种计划的计划靶区(planning target volume,PTV)适形度指数(conformity index,CI)、均匀性指数(homogeneity index,HI)和危及器官的剂量学参数,同时比较2种计划的出束跳数和治疗时间。采用SPSS 19.0软件进行统计学分析。结果:共面VMAT计划与非共面VMAT计划相比,PTV的CI、HI、最大剂量、平均剂量、最小剂量,危及器官如晶体、眼球、脑干等的最大剂量与平均剂量,以及出束跳数在统计学上没有显著性差异(P>0.05);共面VMAT计划的出束时间比非共面VMAT计划减少约7 s,差异具有统计学意义(P<0.05),且非共面VMAT技术涉及转床角时间,进而增加了其治疗时间。结论:对于鼻腔NK/T细胞淋巴瘤的放疗,采用共面VMAT技术比非共面VMAT技术更具优势,因而在鼻腔NK/T细胞淋巴瘤的临床计划设计中推荐使用共面VMAT技术。Objective To discuss the advantages of non-coplanar volumetric modulated arc therapy(VMAT)over coplanar VMAT in the treatment of nasal natural killer/T-cell lymphoma.Methods Ten patients with nasal natural killer/T-cell lymphoma were selected for planning with non-coplanar VMAT and coplanar VMAT on the treatment planning system Raystation 4.7.5.The two plans had the planning target volumes(PTV),conformity indexes(CI),homogeneity indexs(HI),dosimetry parameters of OARs,treatment MUs and time compared,and SPSS 19.0 software was used for statistics analysis.Results There were no statistically significant differences between non-coplanar and coplanar VMATs in CI,HI,max dose,mean dose and min dose of PTV,max and mean doses of OARs such as lens,eyeballs and brainstem as well as in treatment MUs(P>0.05);coplanar VMAT had the delivery time significantly shorter than that of non-coplanar VMAT by 7 s(P<0.05),and non-coplanar VMAT had prolonged treatment time due to couch rotation.Conclusion Coplanar VMAT gains advantages over non-coplanar VMAT,and thus is recommended for the treatment of nasal natural killer/T-cell lymphoma.
