低分子肝素钠联合环孢素A、醋酸泼尼松治疗原发性肾病综合征临床研究

摘　　要：目的分析低分子肝素钠联合环孢素A、醋酸泼尼松在原发性肾病综合征中的疗效。方法选择2016年6月~2019年1月我院收治的原发性肾病综合征患者120例,采用随机数字表法分为两组,对照组使用醋酸泼尼松治疗,观察组增加低分子肝素钠、环孢素A治疗,比较3个月后两组24 h尿蛋白、血肌酐、血清白蛋白、血清总胆固醇水平;记录两组治疗总有效率、不良反应发生率。结果治疗后两组患者24 h尿蛋白、血肌酐、血清总胆固醇水平均降低且观察组低于对照组,两组血清白蛋白均升高且观察组高于对照组。观察组治疗总有效率(91.67%)较对照组(66.67%)高(χ^2=11.07,P=0.00)。观察组不良反应发生率(8.33%)与对照组(5.00%)比较,差异无统计学意义(P>0.05)。结论低分子肝素钠联合环孢素A、醋酸泼尼松治疗原发性肾病综合征能提高疗效,改善肾脏微循环,安全性高,值得推广。Objective To analyze the efficacy of low molecular weight heparin sodium combined with cyclosporin A and prednisone acetate in the treatment of primary nephrotic syndrome.Methods One hundred patients with primary nephrotic syndrome in our hospital from June 2016 to January 2019 were enrolled into the study and divided into two groups with the random number table method.The control group was given prednisone acetate treatment,and the observation group was given low molecular weight heparin sodium combined with cyclosporin A treatment.The levels of 24 h urine protein,serum creatinine,serum albumin,serum total cholesterol were recorded 3 months after treatment between the two groups;the overall response rate of treatment and the incidence of adverse reactions were recorded between the two groups.Results It was found that the levels of 24 h urine protein,serum creatinine serum total cholesterol after treatment in the two groups were all decreased,which were lower in the observation group than in the control group,and the serum albumin levels in the two groups were increased,which were higher in the observation group than in the control group.The overall response rate of treatment in the observation group(91.67%)was higher than that in the control group(66.67%)(χ^2=11.07,P=0.00).There was no statistically significant difference in the incidence of adverse reactions between the observation group(8.33%)and the control group(5.00%)(P>0.05).Conclusion Low molecular weight heparin sodium combined with cyclosporin A and prednisone acetate treatment can improve the efficacy in patients with primary nephrotic syndrome and the renal microcirculation,and has high safety,which is worthy of popularizing.
