表皮移植联合308 nm准分子光治疗稳定期白癜风的临床观察

摘　　要：目的探讨表皮移植联合308 nm准分子光治疗稳定期白癜风的临床疗效。方法选取2018年11月~2019年6月在黑龙江省齐齐哈尔市第一医院皮肤科就诊的白癜风稳定期患者120例,根据患者治疗方案不同分为三组,其中A组患者接受表皮移植治疗,B组患者接受308nm准分子光治疗,C组接受两种方式联合治疗,分析三组患者临床疗效、不同白癜风类型临床疗效、患者不良反应发生情况及治疗满意度。结果 C组总有效率高于A组、B组(P<0.05),A组与B组相比,总有效率无明显差异(P>0.05);A组、B组、C组患者局限性、散发型以及节段型白癜风临床疗效均高于泛发型白癜风患者(P<0.05);三组患者不良反应发生情况比较,差异无统计学意义(P>0.05);C组有满意度高于A组、B组(P<0.05),A组与B组相比,满意度无明显差异(P>0.05)。结论对稳定期白癜风患者采用表皮移植联合308 nm准分子光治疗可以有效提高患者临床疗效,在泛发型白癜风治疗中同样具有较高疗效,不会增加患者不良反应发生率,患者具有较高的满意度,可以在临床中进行进一步推广应用。Objective To explore the clinical observation of epidermic grafting combined with excimer light 308 nm in treatment of vitiligo in stable stage. Methods One hundred and twenty patients with vitiligo in resting stage treated in the Department of Dermatology of The First Hospital of Qiqihar, Heilongjiang from November 2018 to June 2019 were divided into three groups according to the treatment regimen. The patients in group A were given epidermic grafting treatment, the patients in group B were given excimer light 308 nm treatment, and the patients in group C were given epidermic grafting combined with excimer light 308 nm treatment. The clinical efficacy of patients, the clinical efficacy of different types of vitiligo, the occurrence of adverse reactions and treatment satisfaction were analyzed. Results It was found that the effective rate was higher in group C than in group A and group B(P<0.05), without significant differences in the effective rate compared with group A and group B(P>0.05);the clinical efficacy of localized vitiligo,sporadic vitiligo and segmental vitiligo in the three groups were higher than those of generalized vitiligo(P<0.05);there was no statistically significant difference in the occurrence of adverse reactions among the three groups( P>0.05);the degree of satisfaction was higher in group C than in group A and group B(P<0.05), without significant differences in the degree of satisfaction compared with group A and group B(P >0.05). Conclusion Epidermic grafting combined with excimer light 308 nm treatment can effectively improve the clinical efficacy of patients with vitiligo in stable stage,and has the same relatively high efficacy for patients with generalized vitiligo. It will not increase the incidence of adverse reactions, and has relatively high degree of patients’ satisfaction, which can be further popularized and applied in clinical medicine.
