胃收缩力调节系统治疗2型糖尿病及肥胖的研究进展

摘　　要：由于目前大多数的抗糖尿病药物的疗效有限,加之患者对治疗的依从性通常较差,因此,治疗2型糖尿病的新方法仍在不断探索中。本文介绍了一种新研发的治疗2型糖尿病合并超重或肥胖的系统,该系统包含一个植入式脉冲发生器和植入胃浆肌层的引线,脉冲发生器通过此引线来传递电刺激。该系统能识别并自动调节胃的自然电活动,增强胃的收缩力,这种增强的收缩力可以提高迷走神经的信号传入活动,当信号传输到大脑的调节中心时,就会激发典型的饱餐肠道早期反应。据目前所进行的临床试验所示,该系统不仅能改善糖尿病患者的血糖水平,而且对病人的依从性要求也是最低的。同时,随着患者体重的下降,其血压和血脂状况均得到改善。该系统安全可靠,耐受性高,使患者患低血糖的风险较低,这或将成为那些不愿意使用胰岛素或通过减肥手术治疗的肥胖患者的另一种替代性治疗方案。Due to the limited efficacy of most antidiabetic drugs at present and patients’poor compliance with treatment,new therapeutic approaches to type 2 diabetes are still being explored.This review presents a newly developed system for the treatment of type 2 diabetes with overweight or obesity.It includes an implantable pulse generator and leads,and the leads are implanted into the gastric seromuscular layer and transmit electrical stimulation from the generator.The system can discern and automatically regulate the spontaneous electrical activity of the stomach,enhance the contractility of the stomach,and thereby augment the afferent signals of the vagus nerve;when the signals are delivered to the regulatory center of the brain,a typical early gastrointestinal reaction after a full meal will be generated.Current clinical trials have shown that the system can improve the blood glucose level with the least dependence on patients’compliance,and has a low risk of hypoglycemia;at the same time,blood pressure and lipids are improved as the weight of patients decreases.This system can be a safe and reliable alternative therapy for the obese patients who are unwilling to use insulin or undergo weight loss surgery.
