摘 要:目的:比较肾动脉非对比增强流入反转恢复MR血管成像(NC-FIRM-MRA)和增强MR血管成像(CE-MRA)的图像质量,探讨肾动脉NC-FIRM-MRA的临床应用价值。方法:前瞻性收集因病情需要行肾动脉CE-MRA检查的73例患者,同时均行肾动脉NC-FIRM-MRA检查。由2名高年资放射科医师独立阅片,采用双盲法分别对两种不同检查方法的图像行肾动脉图像质量评分及肾动脉分支显示评分,分别采用配对样本的Wilcoxon符号秩检验和χ^2检验进行比较。结果:NC-FIRM-MRA和CE-MRA的肾动脉图像质量评分差异无统计学意义(P> 0.05)。NC-FIRM-MRA和CE-MRA图像的肾动脉分支显示差异有统计学意义(P<0.05)。结论:NC-FIRM-MRA对肾动脉各级分支的显示要优于CE-MRA,而对于肾动脉主干及狭窄的显示两种方法无明显差异。NC-FIRM-MRA成像可作为肾动脉狭窄的检测手段。Purpose: To compare the image quality of renal artery with non-contrast-enhanced inflow reversal and recovery MR angiography(NC-FIRM-MRA) and contrast-enhanced MR angiography(CE-MRA), and to explore the clinical value of renal artery NC-FIRM-MRA. Methods: Seventy-three patients with renal artery CE-MRA were enrolled in the study. Renal artery NC-FIRM-MRA was performed. Two senior radiologists read the films independently, and the renal artery image quality score and the renal artery branch score of images with two different examination methods were evaluated in a double-blind way. Wilcoxon signed rank test and paired samples test were used for the statistical analysis. Results: There was no significant difference in renal artery image quality scores between NC-FIRMMRA and CE-MRA(P>0.05). The display rate of renal artery branches was shown with statistical significant differences between NC-FIRM-MRA and CE-MRA(P<0.05). Conclusion: NC-FIRM-MRA is superior to CE-MRA in the display of renal artery branches, but there is no significant difference between the two methods for renal artery trunk and stenosis. NC-FIRM-MRA scanning can be used as a clinical examination for renal artery lesions.
