摘 要:目的:分析热断层扫描(TTM)成像技术在脑供血不足诊断及筛查中的应用价值。方法:选择在医院进行健康体检的584名脑部TTM检查者资料,并观察其颈椎X射线、颈动脉超声多普勒及头颅核磁共振成像(MRI)等辅助检查情况,以MRI结果为金标准,将584名TTM检查者分为脑供血不足阳性组(103名)和脑供血不足阴性组(481名),分析引起脑供血不足的高危因素,并拟订相关健康管理建议。结果:MRI诊断结果显示,异常103名,正常481名。TTM检查显示,脑供血不足阳性为168名,阴性为416名;TTM检查脑供血不足的灵敏度、特异度和准确率分别为73.79%、80.87%和79.62%;TTM检查脑供血不足阳性与阴性者在肥胖、LDL-C异常、吸烟、酗酒、高血压、颈椎病、多发脑缺血灶及颈动脉硬化等资料上数据对比差异有统计学意义(χ^2=6.352,χ^2=5.339,χ^2=7.161,χ^2=5.214,χ^2=4.342,χ^2=10.423,χ^2=21.904,χ^2=11.102,χ^2=8.697;P<0.05)。结论:TTM检查不仅可以早期发现脑供血不足的状态,还能在形态学检查发现引起脑供血不足的器质性组织结构改变前对脑供血不足发生趋势做出准确评价,是健康管理中对脑供血不足预防干预的重要预警指标。Objective:To analyze the application value of thermal texture maps(TTM)in the diagnosis and screening of insufficient blood supply of cerebrum.Methods:The data of 584 persons who underwent TTM of physical examination for cerebrums were selected.And the X-ray of cervical vertebra,ultrasonic Doppler of carotid artery and magnetic resonance imaging(MRI)of heads of them were compared.According to golden standard(MRI examination),they were divided into positive group(103 cases)and negative group(481 cases)of insufficient blood supply of cerebrum.The highly risk factors that caused insufficient blood supply of cerebrum were analyzed,and the relevant health management suggestions were formulated.Results:The diagnostic results of MRI indicated that 103 cases were abnormal and 481 cases were normal.And the examination results of TTM indicated positive patients with insufficient blood supply of cerebrum were 168 cases and negative patients without that were 416 cases.The diagnostic sensitivity,specificity and accuracy of TTM examination for insufficient blood supply of cerebrum were 73.79%,80.87%and 79.62%,respectively.The differences of clinical data such as obesity,abnormal low-density lipoprotein cholesterol LDL-C,smoking,alcoholism,hypertension,cervical spondylosis,multiple cerebral ischemia and carotid atherosclerosis between positive cases of TTM examination and negative cases of that were significant(χ^2=6.352,χ^2=5.339,χ^2=7.161,χ^2=5.214,χ^2=4.342,χ^2=10.423,χ^2=21.904,χ^2=11.102,χ^2=8.697,P<0.05).Conclusion:TTM can not only find insufficient blood supply on cerebrum at early stage,but also take the accurate evaluation on the occurrent trend of insufficient blood supply of cerebrum before morphological examination finds the structural changes of organic tissues that cause insufficient blood supply of cerebrum.Therefore,TTM is important early-warning indicator for preventing and intervening insufficient blood supply of cerebrum in health management.
