等离子技术治疗变应性鼻炎伴鼻中隔偏曲的远期随访观察研究

摘　　要：目的:研究等离子技术应用于变应性鼻炎(AR)伴鼻中隔偏曲的临床远期疗效。方法:选择在医院治疗的130例伴鼻中隔偏曲AR患者,按随机抽签方式将其分为观察组和对照组,每组65例。对照组采用鼻内镜下鼻中隔黏膜下切除术治疗,观察组在对照组的基础上采用等离子射频消融术联合治疗。比较两组术后1年和2年的远期疗效、复发率及手术前后1年和2年糖精清除时间和鼻腔阻力,比较两组患者术后住院期间并发症情况。结果:观察组术后1年和2年治疗总有效率分别为95.38%和89.23%,显著高于对照组的84.62%和72.31%,差异有统计学意义(χ^2=4.188,χ^2=5.992;P<0.05)。观察组术后1年和2年的糖精清除时间及鼻腔阻力均明显低于对照组,其差异有统计学意义(t糖精清除时间=2.095,t=2.324;t鼻腔阻力=4.407,t=5.356;P<0.05)。观察组复发率显著低于对照组,差异有统计学意义(Log-Rank=7.670,P<0.05)。观察组和对照组并发症总发生率分别为4.62%和9.23%,差异无统计学意义。结论:低温等离子技术应用于AR伴鼻中隔偏曲手术治疗中疗效确切,安全性高,且远期复发率较低。Objective:To study the clinically long-term curative effect of plasma technique in allergic rhinitis(AR)with deviation of nasal septum.Methods:130 patients with AR with deviation of nasal septum who received treatment in hospital were randomly divided into observation group(65 cases)and control group(65 cases).The control group was treated by submucous resection of nasal septum under nasal endoscope,and the observation group was treated by plasma radiofrequency ablation on the basis of the treatment of control group.The long-term curative effect and recurrence rate postoperative 1 year and postoperative 2 years between two groups were compared,respectively.And the clearance time of saccharin and nasal resistance at 1 year and 2 years pre and post operation between two groups were compared.Besides,the complications of patients during hospitalization between two groups were further compared.Results:The total effective rates of one and two years post operation of observation group(95.38%and 89.23%)were significantly higher than those of control group(84.62%and 72.31%)(χ^2=4.188,χ^2=5.992,P<0.05),respectively.The clearance time of saccharin and nasal resistance at one year post operation of observation group were significantly lower than those of control group(tclearance time of saccharin=2.095,t=2.324,tnasal resistance=4.407,t=5.356,P<0.05),respectively.The recurrence rate of observation group was significantly lower than that of control group(Log-Rank=7.670,P<0.05).And Tthe total incidence rate of complications of observation group and control group was were 4.62%and 9.23%,respectively,and the difference of that between the two groups was no significant.Conclusion:The application of low temperature plasma technique in AR with deviation of nasal septum has definite curative effect,and has higher safety.And its long-term recurrence rate is lower.
