颊粘膜含化米索前列醇在足月胎膜早破孕妇促宫颈成熟中的应用

摘　　要：目的观察颊粘膜含化米索前列醇孕妇促宫颈成熟的临床效果、产程及妊娠结局。方法选取2017年1月至2018年4月在聊城市东昌府区妇幼保健院住院分娩的单胎足月胎膜早破初产妇122例,分为颊粘膜含化米索前列醇组(研究组)和缩宫素组(对照组),每组各61例。分别应用颊粘膜含化米索前列醇和缩宫素促宫颈成熟,比较两组产妇的宫颈成熟度、分娩方式、产程及妊娠结局情况。结果2组在宫颈成熟有效率、阴道分娩率、产程方面比较,差异有统计学意义(P<0.05),研究组宫颈成熟有效率、阴道分娩率高于对照组;研究组产程短于对照组;两组绒毛膜羊膜炎比较,差异有统计学意义(P<0.05),研究组小于对照组;2组在产后出血、胎儿窘迫、新生儿窒息方面比较,差异无统计学意义(P>0.05)。结论足月胎膜早破孕妇应用颊粘膜含化米索前列醇可提高宫颈成熟有效率,提高经阴分娩成功率,有效缩短产程,降低感染率,安全性高,效果优于缩宫素。Objective To observe the clinical effect,labor and pregnancy outcome of cervical ripening in pregnant women with buccal mucosa containing misoprostol.Methods 122 cases of fetal premature rupture of membranes were divided into buccal mucosa containing misoprostol(study group)and oxytocin group(control group)in 61 cases in each group from January 2017 to April 2018 in our hospital.The buccal mucosa containing misoprostol and oxytocin were used to promote cervical ripening,and the cervical maturity,mode of delivery,labor and outcome of pregnancy were compared between the two groups.Results There were statistically significant differences between the two groups in the efficiency of cervical maturation,vaginal delivery rate and stage of labor(P<0.05).The study group was smaller than the control group,and there was no statistically significant difference between the two groups in postpartum hemorrhage,fetal distress and neonatal asphyxia(P>0.05).Conclusion The use of buccal mucosa-containing misoprostol can improve the efficiency of uterine cervix maturation,improve the success rate of vaginal delivery,shorten the stage of labor,reduce the infection rate,and have higher safety and better effect than oxytocin.
