慢性乙型肝炎患者外周血T淋巴细胞程序性死亡受体1表达与HBV-DNA水平的相关性

摘　　要：目的分析慢性乙型肝炎(CHB)患者外周血T淋巴细胞程序性死亡受体1(PD-1)表达与乙型肝炎病毒DNA(HBV-DNA)水平的相关性。方法将四川省遂宁市中心医院2016年1月至2019年3月收治的92例CHB患者作为研究对象,另选择60例健康志愿者作为对照组,采用流式细胞仪检测两组受检者外周血CD4+、CD8+T淋巴细胞PD-1表达情况,采用荧光定量PCR检测CHB患者HBV-DNA水平,根据HBV-DNA水平将患者分为HBV-DNA高水平组、HBV-DNA中水平组和HBV-DNA低水平组,比较3组外周血CD4+、CD8+T淋巴细胞PD-1表达情况,分析CHB患者PD-1表达水平与HBV-DNA水平的相关性。结果观察组外周血CD4+、CD8+T淋巴细胞PD-1表达水平高于对照组(P<0.05);不同HBV-DNA水平的CHB患者外周血CD4+、CD8+T淋巴细胞PD-1表达水平:HBV-DNA高水平组>HBV-DNA中水平组>HBV-DNA低水平组(P<0.05);CHB患者外周血CD4+、CD8+T淋巴细胞PD-1表达水平与HBV-DNA水平均呈正相关(r=0.527,r=0.568,P<0.05)。结论CHB患者外周血CD4+、CD8+T淋巴细胞PD-1表达水平较高,且与HBV-DNA水平均呈正相关。Objective To analyze the correlation between the expression of programmed death receptor 1(PD-1)in the peripheral blood T lymphocytes and the level of hepatitis B virus DNA(HBV-DNA)in patients with chronic hepatitis B(CHB).Methods 92 patients with CHB admitted to the hospital between January 2016 and March 2019 were selected as the subjects.60 healthy volunteers were selected as the control group.The expression of PD-1 in peripheral blood CD4+and CD8+T lymphocytes were detected by flow cytometry,and the HBV-DNA level in the patients with CHB was detected by real-time PCR.The patients were divided into the high HBV-DNA level group,middle HBV-DNA level group and low HBV-DNA level group according to the HBV-DNA level.The expressions of PD-1 in peripheral blood CD4+and CD8+T lymphocytes were compared among the three groups,and the correlation between the expression level of PD-1 and HBV-DNA level in patients with CHB were analyzed.Results The expression level of PD-1 in peripheral blood CD4+and CD8+T lymphocytes of the observation group was higher than that in the control group(P<0.05).The order of the expression level in patients with different HBV-DNA levels from high to low was as follows:high HBV-DNA level group,middle HBV-DNA level group,low HBV-DNA level group(P<0.05).The expression level of PD-1 in peripheral blood CD4+and CD8+T lymphocytes was positively correlated wit the HBV-DNA level in patients with CHB(r=0.527,r=0.568,P<0.05).Conclusion The expression level of PD-1 in peripheral blood CD4+and CD8+T lymphocytes of patients with CHB is high and it is positively correlated with the HBV-DNA level.
