经尿道前列腺等离子双极电切治疗良性前列腺增生症的可行性及对患者生活质量的影响

摘　　要：目的分析经尿道前列腺等离子双极电切(TUPKP)治疗良性前列腺增生症(BPH)的可行性及对患者生活质量的影响。方法选取2016年1月~2018年1月来我院就诊的57例BPH患者为研究对象,采用随机数字表法分为实验组27例,对照组30例。实验组采用TUPKP术式治疗;对照组采用传统经尿道前列腺电切术(TURP)进行治疗。比较两组手术时间、术中出血量、留置尿管时间、住院时间、术前及术后1个月国际前列腺症状评分(IPSS)、生活质量(QOL)评分、最大尿流率(QMAX)、并发症总发生率。结果实验组手术时间短于对照组,差异有统计学意义(P<0.05);实验组术中出血量少于对照组,差异有统计学意义(P<0.05);两组的留置尿管时间、住院时间、患者术前及术后1个月IPSS、QOL评分、QMAX比较,差异无统计学意义(P>0.05);实验组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论TUPKP治疗BPH效果确切,对患者生活质量改善效果更为显著。Objective To analyze the feasibility of transurethral plasmakinetic resection of prostate(TUPKP)treating benign prostatic hyperplasia(BPH)and its influence on the quality of life of patients.Methods A total of 57 patients with BPH who came to our hospital from January 2016 to January 2018 were selected as research objects and divided into the experimental group with 27 cases and the control group with 30 cases by random number table method.The experimental group was treated by TUPKP.The control group was treated with traditional transurethral resection of prostate(TURP).The changes of operation time,intraoperative blood loss,indwelling catheter time,hospitalization time,international prostate symptom score(IPSS),quality of life(QOL)score,maximum urinary flow rate(QMAX),total complication incidence rate and so on were compared between the two groups.Results The operation time of the experimental group was shorter than that of the control group,the difference was statistically significant(P<0.05),the amount of bleeding in the experimental group was less than that of the control group,the difference was statistically significant(P<0.05).There were no significant differences in indwelling catheter time,hospitalization time,IPSS,QOL scores and QMAX between the two groups before and one month after operation(P>0.05).The total incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion TUPKP is effective in the treatment of BPH,especially in improving the quality of life of patients.
