基于康复临床融合模式的康复医院工伤医疗控费管理探讨

摘　　要：目的:分析北京某三级康复医院推进康复临床融合模式前后工伤医疗费用情况,探讨三级康复医院工伤控费措施的可行性。方法:以2015年1月~2018年5月出院工伤患者住院费用作为研究对象,对比分析康复临床融合模式前后住院费用相关指标的变化,采用单因素方差分析进行数据分析。结果:2015-2018年该院工伤患者月均出院人次逐年增多(P<0.05),平均住院日逐年下降(P<0.05);2016-2018年出院工伤患者次均费用、药占比逐年下降(P<0.05),耗占比保持稳定。结论:在三级康复医院,实施康复临床融合模式,可有效控制工伤费用的增长,使费用结构趋于合理,促进医疗资源合理利用。Objective:To analyze the cost change of work-related injury hospitalization before and after integration service mode of the rehabilitation and clinical medicine in a Rehabilitation Hospital in Beijing,and to explore the feasibility of the management method of medical fees of work-related injury for a Rehabilitation Hospitals.Methods:The hospitalization expenses of work-related injury in-patients from January 2015 to May 2018 were analyzed.A one-way analysis of variance was used.Results:The monthly discharged number of patients increased from 2015 to 2018(P<0.05),and the average hospitalization days decreased yearly(P<0.05).The proportion of medicine and the consumption of medical materials were increased(P<0.05)by comparing 2016 and 2015.From 2016 to 2018,the average cost and proportion of medicines decreased significantly yearly(P<0.05).The consumption of medical materials remained stable(P>0.05).Conclusion:In the Rehabilitation Hospital,the integration service mode of the rehabilitation and clinical medicine can effectively control the growth of the work-related injury costs,and the cost structure tends to be reasonable,which can ensure the rational use of medical resources.
