中医从肺疫论治传染性肺炎的思路

摘　　要：传染性肺炎的中医病名当为肺疫,发病有即发、伏发两种形式,分寒湿肺疫和温热肺疫两种类型。肺疫初起邪在膜府,继而邪毒内陷于肺,肺失宣肃,痰湿内生,邪毒与痰搏结,壅滞于肺,肺气郁闭。邪伏期根据体质辨证用药,以益气养阴、透邪解毒为主。发病后当顺肺之性,以化痰止咳为基本治法;恢复期正虚邪恋、肺脾两虚,治以益气养阴、透泄余邪为主。总体辨寒热虚实、轻重缓急,灵活应用诸法。Infectious pneumonia is called lung pestilence in traditional Chinese medicine.There are two types of infectious pneumonia:instant attack and latent attack.There are two kinds of cold and damp lung pestilence and warm and hot lung pestilence.At the beginning of the lung pestilence,the pathogenic factors were in the membrane,then the pathogenic toxin was deep in the lung.The lung was lost in dredging and purification,and the phlegm dampness was generated internally.The pathogenic toxin and the phlegm were intermingled,and stagnated in the lung,leading to lung Qi depressed and blocked.In the period of hidden pathogen,according to the syndrome differentiation of the constitution,the medication was mainly used to nourish Qi and Yin,penetrate pathogenic factors and detoxify.After the onset of the disease,the basic treatment methods are to treat the nature of the lung,to penetrate pathogenic factors and detoxify,to diffuse and purify the lung,to dissipate phlegm and stop coughing through differentiating deficiency and excess,cold and heat,and order of priority.In the recovery period,there was lingering pathogen due to deficient vital Qi and deficiency of both the lung and spleen.The treatment should be mainly benefiting Qi and nourishing Yin,and eliminating remaining pathogens.
