国产与进口耗材用于回收式自体输血对回收红细胞质量影响的比较

摘　　要：目的比较国产与进口耗材用于回收式自体输血对异位妊娠破裂急诊手术中回收红细胞质量影响的异同,为临床合理选择相关耗材提供依据.方法选择40例估计出血量>500ml的急诊异位妊娠手术患者,将患者使用随机数字法分别采用国产(北京京精医疗设备公司)和进口(美国haemonetics公司)一次性耗材对腹腔内出血进行回收、洗涤,对不同耗材离心洗涤出来即将回输的血红细胞质量进行比较,并将前后两套耗材离心洗涤的红细胞样本与患者本身静脉血红细胞质量进行检测比对.在显微镜下观察红细胞形态,计算异常形态红细胞的百分比,测定红细胞体积压积、红细胞内外pH值、红细胞内Ca2+浓度,观察回收血液流变学指标(红细胞最大变形指数、聚集指数)等三方面评价红细胞质量.结果国产组和进口组耗材回收的红细胞中异常形态红细胞百分比差异无统计学意义(P>0.05).两组红细胞压积和红细胞质量差异也无统计学意义(P>0.05).国产组和进口组耗材回收的红细胞最大变形指数、聚集指数差异无统计学意义(P>0.05).结论国产和进口耗材回收洗涤的红细胞质量比较差异无统计学意义,均符合临床要求,但国产耗材费用低,性价比高,值得推广.Objective To evaluate the effects of disposable consumables of recyclable self—transfusion equipment,Beijing Jingjing Medical Equipment Company(made in China)and American Haemonetics Company(imported),on erythrocyte quality in emergency ectopic pregnancy surgery.Methods 40 cases of emergency ectopic pregnancy patients were selected.The blood collected from the same operation field was randomly collected and washed with different consumables.The quality of red blood cells which were washed out by centrifligation and were about to be transfused was studied.The quality of red blood cells washed by centrifugation and washed by two sets of consumables was compared with that of the patients themselves.The quality of red blood cells was evaluated,the morphology of red blood cells was observed under the microscope and the percentage of abnormal red blood cells was calculated,testing hematocrit,PH value inside and outside red blood cells,Ca2+concentration in red blood cells,and the recovery hemorheological indexes(maximum deformability index and aggregation index of red blood cells)were observed.Results There was no significant difference in percentage of erythrocyte abnormal morphology between domestic and imported consumables(P>0.05).There was no significant difference in hematocrit and function between the two groups(P>0.05).There was no significant difference in maximum deformability index and aggregation index betAveen domestic and imported consumables(P>0.05).Conclusion There is no statistical difference in the quality of erythrocyte washed by domestic and imported consumables.
