1554例超重/肥胖人群的中医体质类型分析及临床防治思路探索

摘　　要：目的:通过分析超重/肥胖人群的中医体质分布规律及成因,探索超重/肥胖的中医预防治疗思路及健康管理模式。方法:选择2017年1月—2018年12月黑龙江中医药大学附属第一医院治未病中心收治的体检人员2988例,用标准化中医体质辨识量表判断中医体质类型,采用SPSS22.0进行数据分析,得出超重和肥胖相关性较高的中医体质类型。结果:该样本中超重和肥胖人群共1554例(52.01%,1554/2988),其中平和质455例(29.28%,455/1554),偏颇体质1099例(70.72%,1099/1554);偏颇体质中包括单一偏颇体质448例(40.76%,448/1099),兼夹体质651例(59.24%,651/1099);超重人群的中医体质类型以气虚质、阳虚质为主,分别为42.36%(405/956)和32.22%(308/956);肥胖人群的中医体质类型以气虚质、湿热质为主,分别为38.13%(228/598)和36.12%(216/598);随着年龄的增长超重与肥胖者所占的比重呈现逐渐增长的趋势;男性超重与肥胖的比例是女性的1.30倍和2.56倍。结论:超重和肥胖的发病率较高,中医体质类型以偏颇体质为主,且多为兼夹体质。气虚质、阳虚质、湿热质是与超重和肥胖密切相关的中医体质类型,男性超重和肥胖发病率明显高于女性。Objective:To analyze the distribution law of TCM body constitution in the obese and the cause of overweight,thus to explore the prevention and treatment ideas and health management mode of overweight in TCM.Methods:2,988 people who took the physical check-up from January 2017 to December 2018 in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were included in the study.The body constitution of them was identified by using standardized Traditional Chinese Medicine Physique Identification Quantity,and the data were analyzed by spss22.0 trying to find out the body constitution closely related to overweight and obesity in the population.Results:There were 1,554 cases of overweight and obesity in the population(52.01%),among which,455 cases of moderation physical type(29.28%);1,099 cases with biased constitution(70.72%),including 448 cases with single biased body constitution(40.76%)and 651 cases with concurrent body constitution(59.24%).The body constitution types of the overweight people were mainly Yang-deficiency and Qi-deficiency,which accounted for 42.36%and 32.22%respectively.The body constitution types of the obese people were mainly Qi-deficiency and dampness-heat,which accounted for 38.13%and 36.12%respectively.Overweight and obesity showed increasing trend with age.The male were 1.30 times and 2.56 times more likely to be overweight or be obese than the female.Conclusion:The incidence of overweight and obesity is higher,and the body constitution is mainly biased constitution in TCM,and most are concurrent constitution.Qi-deficiency,Yang-deficiency and dampness-heat are main physical types of overweight and obesity.The incidence rate of overweight and obesity is significantly higher in the male than in the female.
