摘　　要：目的:观察脊痛消胶囊对腰椎间盘突出症(LDH)患者的临床疗效及对患者血清中白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)因子的影响。方法:选择符合纳入标准的60例LDH患者,采用随机数表法分为治疗组和对照组,每组30例。治疗组口服脊痛消胶囊,对照组口服根痛平颗粒。比较两组治疗前后视觉模拟评分(VSA)、日本骨科协会评估治疗分数(JOA)及患者血清中IL-1β、IL-6、TNF-α水平的变化。结果:治疗组总有效率为93.33%(28/30),对照组总有效率为70%(21/30),差异有统计学意义(P<0.05)。治疗后两组VAS评分均低于治疗前、JOA评分均高于治疗前,差异有统计学意义(P<0.01),且治疗后治疗组患者VAS评分明显低于对照组、JOA评分明显高于对照组,差异具有显著统计学意义(P<0.01);治疗后两组患者血清中IL-1β、IL-6和TNF-α水平均有明显下调(P<0.01),且治疗组优于对照组(P<0.01)。结论:脊痛消胶囊治疗LDH效果较好,能有效减轻患者疼痛,改善患者腰部功能,其作用机制可能与下调IL-1β、IL-6、TNF-α水平有关。Objective:To observe the clinical effect of Jitongxiao Capsule in the treatment of lumbar disk herniation(LDH)and its influence to the serum levels of IL-1β,IL-6 and TNF-α.Methods:60 LDH patients were randomly divided into the control group and the therapy group,with 30 cases in each group.The therapy group was treated with Jitongxiao Capsule,whereas the control group was treated with Gentongping Capsule.VAS scores,JOA scores,serum levels of IL-1β,IL-6 and TNF-αwere compared before and after the treatment in the two groups.Results:The clinical efficiency of the therapy group was 93.33%(28/30),which was significantly higher than 70%(21/30)of the control group(P<0.05).After the treatment,the VAS scores were lower and the JOA scores were higher than those before the treatment in the two groups(P<0.01);of which the improvements in the therapy group were more significant than those in the control group(P<0.01).The serum levels of IL-1β,IL-6 and TNF-αwere significantly down-regulated(P<0.01),and the down-regulation in the therapy group was more significant than that in the control group(P<0.01).Conclusion:The treatment of LDH with Jitongxiao Capsule is effective,it can effectively alleviate the pain and improve the waist function of the patients.The mechanism may be related to the down-regulation of IL-1β,IL-6 and TNF-α.
