健脾益肾化瘀通络方治疗移植肾功能延迟的疗效观察

摘　　要：目的:探讨健脾益肾化瘀通络方治疗移植肾功能延迟的效果及对TNF-α、IL-6、hs-CRP等炎性反应因子的影响。方法:选取2013年4月至2018年3月武汉科技大学附属普仁医院收治的移植肾功能延迟恢复的患者60例作为研究对象。按照随机数字表法随机分组法分为对照组和观察组,每组30例。对照组给予血液透析联合免疫抑制方案辅助治疗,观察组在对照组基础上给予健脾益肾化瘀通络方治疗。比较2组肾功能指标(Scr、BUN、Ccr)、炎性反应因子(TNF-α、IL-6、hs-CRP)、肾功能恢复时间及临床疗效。结果:治疗后,2组Scr、BUN、TNF-α、IL-6、hs-CRP水平均降低,Ccr水平均升高(P<0.05);观察组治疗后Scr、BUN、TNF-α、IL-6、hs-CRP水平均降低幅度及Ccr升高幅度均高于对照组(P<0.05);观察组治疗后肾功能恢复时间早于对照组(P<0.05);2组患者总有效率比较(观察组96.67%:对照组90.00%)(P>0.05)。结论:健脾益肾化瘀通络方可显著改善移植肾功能水平,缩短移植肾功能延迟恢复的时间,降低血清TNF-α、IL-6、hs-CRP等炎性反应因子水平,临床治疗效果显著,可进一步推广。Objective:To explore the effects of Jianpi Yishen Huayu Tongluo Formula on delayed renal transplantation and its influence on inflammatory factors such as TNF-α,IL-6 and hs-CRP.Methods:A total of 60 patients with delayed recovery of renal transplantation admitted to Puren Hospital Affiliated to Wuhan University of Science and Technology from April 2013 to March 2018 were selected as the study subjects and were divided into a control group and an observation group by random number table method,with 30 cases in each group.The control group was treated with hemodialysis combined with immunosuppressive therapy,while the observation group was treated with Jianpi Yishen Huayu Tongluo Formula on the basis of the control group.The indexes of renal function(Scr,BUN,Ccr),inflammatory factors(TNF-α,IL-6,hs-CRP),recovery time of renal function and clinical efficacy were compared between the 2 groups.Results:After treatment,the levels of Scr,BUN,TNF-α,IL-6,hs-CRP in the observation group decreased and Ccr increased(P<0.05);The levels of Scr,BUN,TNF-α,IL-6 and hs-CRP in the observation group were lower than those in the control group,and Ccr increase was higher than that in the control group(P<0.05);The recovery time of renal function in the observation group was earlier than that in the control group(P<0.05);The total effective rate between the 2 groups was as follows(observation group:96.67%:control group:90.00%)(P>0.05).Conclusion:Jianpi Yishen Huayu Tongluo Formula can significantly improve the level of transplanted kidney function,shorten the time of delayed recovery of transplanted kidney function,and reduce the levels of inflammatory factors such as TNF-alpha,IL-6,hs-CRP in serum.The clinical therapeutic effects are remarkable and can be further promoted.
