托特罗定联合BTX-A治疗女性膀胱过度活动症的疗效观察

摘　　要：目的评价托特罗定联合膀胱内注射A型肉毒素治疗女性膀胱过度活动症(OAB)的临床疗效及其安全性.方法2016年6月至2018年10月OAB女性患者72例,随机分为4组,各18例.A组:口服安慰剂、B组:口服托特罗定片2mg,1次/d,共4周,C组:膀胱内注射A型肉毒素,D组:膀胱内注射A型肉毒素后并立即开始服用托特罗定片2mg,1次/d,分别记录治疗前后4周平均每日日间排尿次数、平均每次排尿量、夜尿次数、尿急次数、初始尿意膀胱容量、最大膀胱容量、OABSS评分、QOL评分情况.结果治疗后D组平均每日日间排尿次数、平均每次排尿量、夜尿次数、尿急次数、初始尿意膀胱容量、最大膀胱容量、OABSS评分、QOL评分均较其他组有明显改善.结论口服托特罗定联合膀胱内注射A型肉毒素治疗OAB疗效确切,安全性高.Objective To evaluate the efficacy and safety of bladder injections of botulinum toxin type A combined with oral Tolterodine in female patients with overactive bladder.Methods The female patient with age range 18 to 55 years old who underwent OAB in our hospital from June 1st 2016 to October 1st 2018,72 cases were divided into 4 groups,group A(placebo group),cases of group B accepted Tolterodine(2mg)once a day for 4 weeks,cases of group C accepted botulinum toxin type A injection in bladder,cases of group D accepted both botulinum toxin type A injection in bladder and Tolterodine orally.The following parameters were recorded to evaluate the effects 4 weeks later:average daytime urination frequency,urination frequency at night,a average quantity of each urination,frequency of urgency,bladder capacity at first desire,maxim a bladder capacity,OABSS scores and QOL scores.Results After 4-week treatment with botulinum toxin type A,all parameters,a average daytime urination frequency,urination frequency at night,average quantity of each urination,frequency of urgency,bladder capacity at first desire,maxim a bladder capacity,OABSS scores and QOL scores in cases of group D had a better improvement than those of cases of the other groups.Conclusion Oral Tolterodine combined with injection of botulinum toxin type A is an effective and safety treatment for overactive bladder.
