摘　　要：目的分析超声心动图(UCG)与CT血管造影(CTA)在复杂性先天性心脏病(CCHD)中的诊断价值。方法收集52例CCHD患者的临床资料,所选患者均经手术证实,术前均行UCG及CTA,对比分析UCG、CTA及两者联合的诊断准确率。结果经手术证实本组共存在166处心脏畸形。UCG共检出148处,准确率为89.2%,CTA共检出154处,准确率为92.8%,略高于UCG,但差异无统计学意义(P>0.05)。UCG与CTA联合共检出163处,准确率98.2%,联合诊断准确率高于UCG及CTA,差异有统计学意义(P<0.05)。结论UCG操作简单、费用低廉、可重复性强,可作为CCHD的首选筛查方法,CTA对大血管的显示更为直观,对心外结构的检出也优于UCG,但CTA具有一定电离辐射。对临床怀疑CCHD但UCG阴性患者,应联合应用CTA,可有效提高术前检出率。Objective To analyze the diagnostic value of echocardiography(UCG)and CT angiography(CTA)in complex congenital heart disease(CCHD).Methods Clinical data of 52 patients with CCHD were collected.All the patients were confirmed by surgery,and UCG and CTA were performed preoperatively.The diagnostic accuracy of UCG,CTA,and their combination were compared and analyzed.Results 166 heart malformations were found in this group.148 malformations were detected by UCG with the accuracy of 89.2%,and 154 malformations were detected by CTA with the accuracy of 92.8%,which was slightly higher than that of UCG,but there was no statistically significant difference(P>0.05).A total of 163 malformations were detected by UCG combined with CTA,with the accuracy of 98.2%,which was higher than that of UCG and CTA,with statistically significant difference(P<0.05).Conclusions UCG is simple,inexpensive,and repeatable,and can be used as the first screening method for CCHD.CTA is more intuitive in the display of great vessels and better in the detection of extracardiac structures than UCG.However,CTA has certain ionizing radiation.For patients with clinical suspicion of CCHD but UCG negative,CTA should be applied combinedly to effectively improve the preoperative detection rate.
