摘　　要：目的探讨家庭延伸护理联合自我管理教育在慢性心衰(CHF)患者跟踪随访中的应用效果。方法选取2016年03月~2018年05月我院收治的104例CHF患者,随机将其分为2组,各52例。对照组进行自我管理教育,观察组加以家庭延伸护理。干预1年后,对比两组患者自我管理能力、心脏事件发生率和再次住院率。结果干预后,观察组日常生活管理能力和规范化服药率均高于对照组,心脏事件发生率和再次住院率比对照组低,差异有统计学意义(P<0.05)。结论家庭延伸护理联合自我管理教育能提高CHF患者自我管理能力,降低心脏事件发生率和再次住院率。Objective To investigate the effect of extended family care combined with self-management education in the follow-up of patients with chronic heart failure(CHF).Methods 104 cases of CHF patients from March 2016 to May 2018 in our hospital were randomly divided into control group and observation group,each with 52 cases.Two groups were given self-management education and the observation group was additional with extended family care.The self-management ability,the incidence of cardiac event and rehospitalization rate were compared between the two groups after one year follow-up.Results The ability of daily-life management and normalized dosing incidence of the observation group were higher than those of the control group,while the incidence of cardiac event and rehospitalization rate in the control group were lower than those in the observation group,and the difference was statistically significant(P<0.05).Conclusion Extended family care combined with self-management education can enhance patients’ability of self-management,and decrease the incidence of cardiac event and rehospitalization rate.
