摘  要:目的利用中药复方本身具有的多途径、多环节、多靶点的特点,探讨壮肝逐瘀煎及其不同拆方组合对肝纤维化模型大鼠微循环的调控。方法采用四氯化碳(CCl4)复合因素大鼠肝纤维化模型,观察各组大鼠肝纤维化情况;用酶联免疫吸附法(ELISA)检测血清中血管活性物质[内皮素1(ET-1)、一氧化氮(NO)、诱导型一氧化氮合酶(iNOS)的含量;用RT-PCR技术检测各组VEGF、PDGF的含量;应用超声微泡造影评估各组大鼠肝脏微循环、微血管血流状态和速度。结果①C组及不同组合分组和G组均能不同程度改善肝纤维化大鼠肝脏病理组织、减轻大鼠肝纤维化的程度,与G组相比,有统计学意义;②与正常对照组相比,病理模型组中各给药组对ET-1、NO、iNOS含量均有明显的影响(P<0.01或P<0.05),肝纤维化程度越高,ET-1、NO、iNOS的含量越高;VEGF、PDGF的含量也越高;各用药组HVAT、HA-HVTT均有不同程度的延长(P<0.01)。③与病理模型组比较,各用药组血清ET-1、NO、iNOS含量均有不同程度的降低(P<0.05),其中C组(壮肝逐瘀煎全方组)下降最为明显;各用药组VEGF、PDGF的表达量均下降(P<0.05),而C组与G组(大黄?虫丸组)比较(P>0.05),无明显差异;各用药组HVAT、HA-HVTT均有不同程度的延长(P<0.01),其中以C组明显。Objective Based on the characteristics of multi-channel,multi-link and multi-target of traditional Chinese medicine compounds,the regulation of Zhuanggan Zhuyu Decoction and its different combinations on the microcirculation of rat model of liver fibrosis was discussed. Method The hepatic fibrosis of rats in each group was observed by carbon tetrachloride(CCl4)composite factor model. Enzyme linked immunosorbent assay(ELISA) was used to detect the contents of vasoactive substances(endothelin,ET-1,nitric oxide,NO,inducible nitric oxide synthase,iNOS). Vascular endothelial growth factor(VEGF) and platelet-derived growth factor(PDGF) in each group were determined by real-time RT-PCR. Hepatic microcirculation,microvascular blood flow velocity and blood flow were assayed by contrast-enhanced ultrasoundgraphy. Results ①C group and different combinations group and G group can improve hepatic fibrosis rat liver fibrosis to varying degrees,reduce the degree of liver fibrosis in rats. Compared with the G group,there was statistical significance. ②Compared with the normal control group,the contents of ET-1,NO and iNOS in the pathological model group were significantly affected by each of the drug groups(P<0.01 or P<0.05). The higher the degree of liver fibrosis,the higher the contents of ET-1,NO and iNOS. The contents of VEGF and PDGF were also higher,and HVAT and HA-HVTT were extended to different degrees in each treatment group((P<0.01). ③Compared with the model group,the levels of ET-1,NO and iNOS in the serum of all the groups were decreased to different degrees,especially those in the C group. VEGF and PDGF levels were decreased in all groups(P<0.05),while there was no statistical difference between C group and G group(P>0.05). HAVT and HA-HVTT were prolonged in different degrees in each group,especially those in group C.
