摘 要:目的:观察电针百会穴对内淋巴积水(EH)豚鼠听觉脑干诱发电位(ABR)、耳蜗电图(EcochG)及耳蜗血流灌注量(CoBF)的影响。方法:采用腹腔注射精氨酸加压素诱导EH豚鼠模型。造模组豚鼠100只随机分为模型组、电针穴位组(电针百会穴)、电针非穴位组(电针股部非穴位)、药物组(灌胃血管加压素拮抗剂OPC-41061)和针药联合组(电针百会穴+OPC-41061灌胃),每组20只。另设20只正常豚鼠为对照组。对照组和模型组豚鼠不予干预,其他各组分别进行相应干预。治疗28 d后,采用Navigator PRO听觉脑干诱发电位诊断系统检测豚鼠ABR和EcochG,采用激光多普勒血流仪检测CoBF。结果:①与对照组相比,模型组在各频率的ABR阈值均明显升高(P<0.05)。与模型组相比,电针穴位组在250、500、1 000、4 000、6 000、8 000 Hz频率下的ABR阈值均显著降低(P<0.05),电针非穴位组在4 000、6 000、8 000 Hz频率下的ABR阈值亦显著降低(P<0.05),药物组和针药联合组在各频率的ABR阈值均明显降低(P<0.05)。与电针穴位组相比,药物组在1 000和8 000 Hz频率的ABR阈值明显降低(P<0.05),针药联合组在500、1 000、4 000、8 000 Hz频率的ABR阈值明显降低(P<0.05)。针药联合组与药物组在各频率的ABR阈值比较,差异均无统计学意义(P>0.05)。②在8 000 Hz频率下,模型组-SP/AP值较对照组显著升高(P<0.05);与模型组相比,电针穴位组、药物组和针药联合组的-SP/AP值显著降低(P<0.05);电针非穴位组的-SP/AP值明显高于电针穴位组(P<0.05)。电针穴位组与药物组、针药联合组比较,差异均无统计学意义(P>0.05)。③模型组豚鼠的CoBF较对照组明显降低(P<0.05);与模型组相比,电针穴位组和针药联合组CoBF显著升高(P<0.05)。电针穴位组与药物组、针药联合组比较,差异均无统计学意义(P>0.05)。结论:电针百会穴可明显改善EH豚鼠的听力和电生理,其作用可能与增加CoBF有关。Objective: To observe the effects of acupuncture at‘Baihui’( GV20) acpoint on auditory brainstem response( ABR),electrocochleogram( EcochG) and cochlear blood flow( CoBF) in guinea pigs with endolymphatic hydrops( EH). Methods: The EH model in guinea pig was induced by intraperitoneal injection of arginine vasopressin( AVP). One hundred guinea pigs in the modeling group were randomly divided into the model group,electroacupuncture acupoint group( electroacupuncture at ‘Baihui’ acpoint),electroacupuncture non-acupoint group( electroacupuncture at femoral non-acupoint),drug group( intragastric administration of AVP antagonist OPC-41061) and combination group( electroacupuncture at‘Baihui’acpoint + OPC-41061 intragastric administration),20 guinea pigs in each group. Another 20 normal guinea pigs were taken as the control group. No treatment was performed in the control group and the model group,and the other groups were treated with the corresponding intervention for 28 days. The ABR and EcochG of guinea pigs were detected by Navigator PRO auditory brainstem evoked potential diagnosis system,and the CoBF was detected by laser doppler flow meter. Results:(1) Compared with the control group,the ABR threshold of the model group was increased significantly at all frequencies( P<0.05). Compared with the model group,the ABR threshold at 250,500,1 000,4 000,6 000 and 8 000 Hz was significantly decreased in the electroacupuncture acupoint group( P<0.05),the ABR threshold at 4 000,6 000 and 8 000 Hz was also decreased significantly in the electroacupuncture non-acupoint group( P<0.05),and the ABR threshold in the drug group and the combination group was decreased significantly at all frequencies( P<0.05). Compared with the electroacupuncture acupoint group,the ABR threshold at 1 000 and 8 000 Hz in the drug group was significantly decreased( P<0.05),and the ABR threshold at 500,1 000,4 000,8 000 Hz in the combination group was significantly decreased( P<0.05).
