摘 要:目的:观察芪山无糖颗粒对代谢综合征(MS)患者肠道菌群的调节作用。方法:纳入80例MS患者,随机分为二甲双胍组和芪山无糖颗粒组各40例,同时纳入正常对照组40例,二甲双胍组服用二甲双胍片,芪山无糖颗粒组服用芪山无糖颗粒,总干预期为12周。采用传统培养、生化鉴定的方法,变性梯度凝胶电泳技术(DGGE)和荧光定量PCR技术(RT-PCR)技术等,检测和比较2组患者的肠道菌群数量和结构特征。治疗前后检测2组患者的空腹血糖(FBG)、餐后2 h血糖(P2hBG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDC-C)水平。结果:治疗后,2组FBG、P2hBG、HbA1c水平均较治疗前下降(P<0.05),芪山无糖颗粒组FBG、P2hBG、HbA1c水平均低于二甲双胍组(P<0.05)。2组TC、TG、LDC-C水平均较治疗前下降(P<0.05),芪山无糖颗粒组TC、TG、LDC-C水平均低于二甲双胍组(P<0.05)。二甲双胍组肠道菌群丰度低于正常对照组,芪山无糖颗粒组肠道菌群丰度较正常对照组上升。二甲双胍组的肠道菌群与正常对照组的肠道菌群相似度低,芪山无糖颗粒组的肠道菌群与正常对照组具有较高的相似性,趋于接近正常对照组。二甲双胍组的菌群与正常对照组的菌群差异较大,而芪山无糖颗粒组的菌群与正常对照组组的菌群差异较小。芪山无糖颗粒组拟杆菌属减少、乳酸杆菌属增加,二甲双胍组并未表现出有益菌群的增多和减少。结论:芪山无糖颗粒可调节肠道菌群结构,减少拟杆菌属等的数量,增加乳酸杆菌属的数量,对降低血糖、调节血脂,改善肥胖、糖尿病及高血脂等代谢综合征相关症状具有积极意义。Objective: To observe the regulatory effect of Qishan Wutang granules on intestinal flora in patients with metabolic syndrome(MS). Methods:A total of 80 MS patients were enrolled and randomly divided into the metformin group and the Qishan Wutang granules group,40 cases in each group;and another 40 normal patients were enrolled in the control group. The metformin group was treated with metformin tablets,and the Qishan Wutang granules group was treated with Qishan Wutang granules, with a total intervention period of 12 weeks. Methods like traditional culture and biochemical identification, as well as denatured gradient gel electrophoresis(DGGE) and Real-Time Flurogenic Quantitative Polymerase ChainReaction(RT-PCR) were used to detect and compare the number and structural characteristics of intestinal flora in the two groups of patients. Before and after treatment,the levels of fasting plasma glucose(FBG),postprandial plasma glucose at 2 hours(P2 hBG), glycated hemoglobin(HbA1 c), total cholesterol(TC), triglyceride(TG), and low-density lipoprotein cholesterol(LDL-C) were measured. Results:After treatment,the levels of FPG,P2 hBG and HbA1 c in the two groups were decreased when compared with those before treatment(P<0.05),and the above levels in the Qishan Wutang granules group were lower than those in the metformin group(P<0.05). The levels of TC,TG and LDC-C in the two groups were decreased when compared with those before treatment(P<0.05), and the above levels in the Qishan Wutang granules group were lower than those in the metformin group(P<0.05). The diversity of intestinal flora in the metformin group was lower than that in the normal control group,and the diversity in the Qishan Wutang granules group was higher than that in the normal control group. The intestinal flora in the metformin group was slightly similar to that in the normal control group,while the intestinal flora in the Qishan Wutang granules group was highly similar to and tended to approach that in the normal control group.
