葆宫止血颗粒联合米非司酮片治疗围绝经期功能失调性子宫出血临床研究

摘　　要：目的:观察葆宫止血颗粒联合米非司酮片治疗围绝经期功能失调性子宫出血的临床疗效。方法:将106例肾阴虚型患者随机分为对照组和观察组各53例。2组患者均服用米非司酮片,观察组加用葆宫止血颗粒治疗。2组疗程均为6个月经周期。治疗前后评定月经失血图(PBAC)、出血时间、月经周期和肾阴虚证评分,检测血红蛋白(Hb)和子宫内膜厚度。结果:治疗后,观察组临床疗效优于对照组(P<0.05)。2组PBAC、出血时间、月经周期和肾阴虚证评分均较治疗前下降(P<0.01),观察组4项评分均低于对照组(P<0.01)。2组Hb水平均较治疗前升高,子宫内膜厚度均较治疗前减少,差异均有统计学意义(P<0.01)。观察组Hb水平高于对照组,子宫内膜厚度薄于对照组,差异均有统计学意义(P<0.01)。结论:葆宫止血颗粒联合米非司酮片治疗肾阴虚型围绝经期功能失调性子宫出血患者可有效减少出血,改善临床症状,调节月经规律。Objective:To observe the clinical effect of Baogong Zhixue granules combined with mifepristone tablets for dysfunctional uterine bleeding during perimenopause. Methods:Divided 106 cases of patients of kidney-yin deficiency type into the control group and the observation group randomly, 53 cases in each group. Both groups received the oral administration of mifepristone tablets, while the observation group additionally received Baogong Zhixue granules. The treatment of both group lasted for 6 menstrual cycles. Evaluated the scores of pictorial blood loss assessment chart(PBAC),time of bleeding, menstrual cycles and kidney-yin deficiency syndrome in both groups, and measured the level of hemoglobin(Hb) and endometrial thickness before and after treatment. Results: After treatment, the clinical effect in the observation group was better than that in the control group(P<0.05). The scores of PBAC, time of bleeding, menstrual cycles and kidney-yin deficiency syndrome in both groups were lower than those before treatment(P<0.01);each score above in the observation group was lower than those in the control group(P<0.01). The levels of Hb in both groups were higher than those before treatment,and the endometrial thickness was lower than that before treatment,differences being significant(P<0.01). The level of Hb in the observation group was higher than that in the control group,and the endometrial thickness was lower than that in the control group, differences being significant(P<0.01). Conclusion: Baogong Zhixue granules combined with mifepristone tablets for patients with perimenopausal dysfunctional uterine bleeding of kidney-yin deficiency can effectively reduce bleeding,improve clinical symptoms and regulate menstruation.
