新生儿经鼻留置胃管护理现状

摘　　要：在新生儿体重增长过程中,需合理喂养,尤其是早产儿、低体重出生儿,胃肠肌肉组织薄弱,胃肠动力功能不成熟,由于吸吮-吞咽-呼吸功能不协调,易导致呛奶窒息,经口喂养困难,为保证热量及蛋白质供给,管饲喂养属于肠内营养常用治疗方案。为降低胃管非计划性拔管率、防止皮肤损伤,提高患儿舒适度,提高护士工作效率,本文对护理人员在固定方式、降低非计划性拔管率、防止皮肤损伤等方面进行综述。胃管固定方式包括“工”字型、“Y”字型、“T”字型、蝶形等方式;对非计划性拔管原因进行分析统计,开展品管圈活动运用质量管理工具降低非计划性拔管率;采用水胶体敷料保护皮肤后,再用高黏性胶布固定胃管防止皮肤损伤等;以期望降低新生儿胃管非计划拔管率、减轻患儿痛苦,更多地体现人文关怀,使新生儿胃管留置更具人性化和科学性,提高护理质量,保障患儿安全。In the process of newborn weight increase,it is necessary to reasonably feed,especially premature infants and low-weight children,weak gastrointestinal muscle tissue,immature gastrointestinal dynamic function,due to sucking-swallowing-respiratory function is not coordinated,easy to lead to choking milk asphyxia,oral feeding is difficult,In order to ensure the supply of calories and proteins,tubular feeding is a common treatment program for enteral nutrition.In order to reduce the gastric tube unplanned tubing,prevent skin damage,improve the comfort of children,improve the nurse's work efficiency,this article summarized the vast number of nursing workers in fixed mode,reduce unplanned tubing,prevent skin injury and other aspects of discussion and research.Stomach tube fixation includes"G ong"font,"Y"font,"T"font,butterfly shape and so on;The reasons of unplanned tubing were analyzed and the quality management tool was used to reduce the rate of unplanned tubing.The use of hydrogel dressing to protect the skin,and then use high viscosity adhesive tape to fix the gastric tube to prevent skin damage.It is hoped to reduce the rate of unplanned gastric tube extraction of newborns,reduce the pain of children,and more embody humanistic care,so that newborn gastric tube retention is more humane and scientific,improve the quality of care,and ensure the safety of children.
