胰岛素抵抗与老年2型糖尿病患者骨钙素水平的关系

摘　　要：目的探讨胰岛素抵抗与老年2型糖尿病患者骨钙素水平的关系。方法选择2018年1~12月在我院诊断治疗的2型糖尿病患者20例为观察组,另选择同期在我院健康体检者20例为对照组。比较两组FBG、2 h PG、TG、TC、OC、HbA1c、FINS、FCP、HOMA-IR、HOMA-β水平。比较观察组不同骨钙素水平患者上述指标水平。分析老年2型糖尿病患者骨钙素水平与FINS、FCP、HOMA-IR、HOMA-β之间的相关性。结果观察组FBG、2 h PG、TG、HbA1c、HOMA-IR显著高于对照组,差异均有统计学意义(P<0.05);观察组OC、FINS、FCP、HOMA-β显著低于对照组,差异有统计学意义(P<0.05)。糖尿病患者OC≤5.2 ng/mL组FBG、HbA1c水平显著高于>5.2 ng/mL组,差异有统计学意义(P<0.05);FCP显著低于>5.2 ng/mL组,差异有统计学意义(P<0.05)。2型糖尿病患者OC水平与HOMA-IR呈负相关的关系,与FINS、FCP、HOMA-β呈正相关的关系。结论2型糖尿病患者骨钙素水平下降,并且OC水平与胰岛素抵抗有密切的相关性,OC可间接反映2型糖尿病患者胰岛素抵抗情况。Objective To investigate the relationship between insulin resistance and osteocalcin levels in the elderly patients with type 2 diabetes.Methods 20 patients with type 2 diabetes who were diagnosed and treated in our hospital from January to December 2018 were selected as the observation group.Another 20 healthy people who received physical examination in our hospital during the same period were selected as the control group.The levels of FBG,2 h PG,TG,TC,OC,HbA1c,FINS,FCP,HOMA-IR and HOMA-βwere compared between the two groups.The levels of above indicators in the patients with different osteocalcin levels were compared in the observation group.The correlation between osteocalcin levels and FINS,FCP,HOMA-IR and HOMA-βin elderly patients with type 2 diabetes were analyzed.Results The FBG,2hPG,TG,HbA1c and HOMA-IR in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The OC,FINS,FCP and HOMA-βin the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of FBG and HbA1c in the OC≤5.2 ng/mL group were significantly higher than those in the OC>5.2 ng/mL group among the diabetic patients,and the differences were statistically significant(P<0.05);the FCP was significantly lower than that in the>5.2 ng/mL group,and the difference was statistically significant(P<0.05).There was a negative correlation between OC level and HOMA-IR in the patients with type 2 diabetes,and there was a positive correlation between OC level and FINS,FCP and HOMA-β.Conclusion The level of osteocalcin in the patients with type 2 diabetes is decreased,and the level of OC is closely related to insulin resistance.OC can indirectly reflect insulin resistance in the patients with type 2 diabetes.
