摘 要:目的分析三维超声联合磁共振成像检测在胎儿骨骼系统发育异常诊断中的临床效果。方法选取2018年5月~2019年4月我院和广州市妇女儿童医疗中心分娩,且在产后或引产证实胎儿骨骼系统发育异常孕妇61例,均在产前接受三维超声及磁共振检查,跟踪妊娠,与产后跟踪结果相对比,分析产前三维超声、磁共振各异常检出准确率及检查图像差异。结果产前三维超声单独检出50例(81.97%),磁共振产前检查诊断30例(49.18%),三维超声联合磁共振检出60例(98.36%);三维超声+磁共振检出率98.36%高于产前三维超声检查率81.97%,数据差异有统计学意义(P<0.05);三维超声+磁共振检出率98.36%高于磁共振产前检查率49.18%(P<0.05);三维超声检查图像质量优良率75.41%(46/61),图像质量评分(3.41±0.38)分;磁共振检查图像质量优良率96.72%(59/61),图像质量评分(4.75±0.20)分;三维超声检查图像质量优良率低于磁共振,数据差异有统计学意义(P<0.05);三维超声检查图像质量评分低于磁共振,数据差异有统计学意义(P<0.05)。结论产前三维超声、核磁共振在胎儿骨骼发育系统异常诊断中均可发挥一定的效用,就单独检查而言,三维超声检查的临床价值更好,而联合磁共振异常检出准确率好于单独检查。Objective To analyze the clinical effect of 3D ultrasound combined with magnetic resonance imaging in the diagnosis of fetal skeletal dysplasia.Methods Sixty-one pregnant women who delivered in our hospital and Guangzhou Women and Children’s Medical Center from May 2018 to April 2019,and whose babies had confirmed fetal dysplasia after birth or inducted labor were given 3D ultrasound and magnetic resonance imaging before delivery.Pregnancy was followed up.The results were compared with postpartum follow-up results.The abnormality detection accuracy rate and image differences of prenatal 3D ultrasound and magnetic resonance imaging were analyzed.Results Fifty cases(81.97%)were detected by prenatal 3D ultrasound alone,thirty cases(49.18%)were detected by magnetic resonance prenatal examination,and sixty cases(98.36%)were detected by 3D ultrasound combined with magnetic resonance imaging.The detection rate of 3D ultrasound plus magnetic resonance imaging was 98.36%,which was higher than that of prenatal 3D ultrasound,which was 81.97%,with statistically significant difference(P<0.05).The detection rate of 3D ultrasound plus magnetic resonance imaging was 98.36%,which was higher than that of prenatal magnetic resonance imaging,which was 49.18%(P<0.05).The 3D ultrasound had an image quality excellence rate of 75.41%(46/61)and an image quality score of(3.41±0.38)points.Magnetic resonance imaging had an image quality excellence rate of 96.72%(59/61)and an image quality score of(4.75±0.20)points.The image quality excellence rate of 3D ultrasound was higher than that of magnetic resonance,with statistically significant difference(P<0.05).The image quality score of 3D ultrasound was lower than that of magnetic resonance imaging,with statistically significant difference(P<0.05).Conclusion Prenatal 3D ultrasound and magnetic resonance imaging have effect on the diagnosis of fetal skeletal development abnormalities.In terms of single examination,the clinical value of 3D ultrasound is higher.3D ultrasound combined with magn
