参麦注射液联合桂哌齐特治疗慢性充血性心力衰竭临床研究

摘　　要：目的:观察参麦注射液联合桂哌齐特治疗慢性充血性心力衰竭(CHF)的临床疗效。方法:将CHF患者101例随机分为对照组50例,治疗组51例;对照组常规给予桂哌齐特治疗,治疗组在对照组的基础上加用参麦注射液治疗,疗程均为2周;观察记录2组治疗前后血流动力学指标,包括心率(HR)、左心室射血分数(LVFF)、平均动脉压(MAP)、体循环阻力(SVR)的变化情况;记录2组治疗前后血液流变学(全血黏度、血浆黏度、纤维蛋白原、红细胞压积)指标的变化;观察治疗后2组患者总体心功能改善情况。结果:治疗后,心功能改善总有效率治疗组为94.1%,对照组为80.0%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患者HR、MAP、SVR值均较治疗前降低(P<0.05),LVFF值较治疗前升高(P<0.05);且治疗组上述各项指标改善较对照组更显著(P<0.05)。治疗后,2组患者全血黏度、血浆黏度、纤维蛋白原、红细胞压积水平均较治疗前降低(P<0.05),且治疗组上述各项水平均低于对照组(P<0.05)。结论:参麦注射液联合桂哌齐特治疗CHF,可显著降低患者血液黏度,增加左心室射血分数,从而更好地保护患者心脏功能,值得临床推广应用。Objective:To observe the clinical effect of Shenmai injection combined with cinepazide for chronic congestive heart failure(CHF). Methods: A total of 101 cases of CHF patients were randomly divided into the control group and the treatment group,50 and 51 cases in each group respectively. The control group was routinely treated with cinepazide,and the treatment group was additionally treated with Shenmai injection based on the treatment of the control group. Both groups were treated for two weeks. Before and after treatment in the two groups,the changes in hemodynamic indexes including heart rate(HR),left ventricular ejection fraction(LVEF),mean arterial pressure(MAP) and systemic vascular resistance(SVR)were observed and recorded;the changes in hemorheological indexes of including whole blood viscosity,plasma viscosity,fibrinogen and hematocrit were recorded. After treatment,the total improvement of the heart function in the two groups was observed. Results:After treatment,the total effective rate of the heart function improvement was 94.1% in the treatment group,and 80.0% in the control group,the difference being significant(P<0.05). After treatment,the values of HR,MAP and SVR in the two groups were decreased when compared with those before treatment(P<0.05),and the values of LVEF were increased when compared with those before treatment(P<0.05);the improvement of the above indexes in the treatment group was more significant than that in the control group(P<0.05). After treatment, the levels of whole blood viscosity, plasma viscosity, fibrinogen and hematocrit in the two groups were all decreased when compared with those before treatment(P<0.05),and the above levels in the treatment group were all lower than those in the control group(P<0.05). Conclusion: The therapy of Shenmai injection combined with cinepazide in treating CHF can significantly decrease blood viscosity and increase cardiac output,thus can better protect the heart function of patient,which is worthy of clinical promotion and application.
