64排多层螺旋CT冠状动脉成像在冠状动脉血管搭桥及支架植入术后复查中的应用价值

摘　　要：目的探讨64排多层螺旋CT冠状动脉成像在冠状动脉血管搭桥及支架植入术后复查中的应用价值。方法选取丹东市第一医院2015年9月-2018年4月收治的用64排多层螺旋CT进行搭桥术及冠状动脉支架植入术后血管扫描的患者45例,所有患者均进行CT冠状动脉成像,观察多层螺旋CT冠状动脉成像对冠状动脉支架、管腔狭窄的显示情况,并对桥血管的闭塞狭窄进行评价。结果术后冠状动脉多层螺旋CT成像显示,患者共搭桥136支,冠状动脉多层螺旋CT成像对133支桥血管成功显像,显像率为97.79%。45例共置入97枚冠状动脉支架。其中,42枚支架评为3级,43枚支架评为2级,12枚支架评为1级。冠状动脉多层螺旋CT成像检查结果显示,植入的97枚支架中,仅5枚支架不能作为影像学诊断评价依据。结论64排多层螺旋CT冠状动脉成像能够对冠状动脉支架及其管腔狭窄显示良好,以及对冠状动脉的走行进行准确反映,可以用于管冠状动脉血管搭桥及支架植入术后复查中的血管评价。Objective To explore the applicability of 64-slice spiral CT coronary artery imaging in the reexamination after coronary artery bypass grafting and stent implantation.Methods Forty-five patients admitted to our hospital between September 2015 and April 2018 were selected for vascular scanning after bypass surgery and coronary stent implantation with 64-slice spiral CT.All the patients underwent CT coronary angiography to observe the display of coronary stent and lumen stenosis by multi-slice spiral CT coronary angiography and to evaluate the occlusion stenosis of bridge vessels.Results The imaging results showed that these patients had a total of 136 bridges,and the imaging rate of 133 bridges was 97.79%.A total of 97 coronary stents were implanted in 45 cases of tumor.Among them,42 stents were rated grade 3,43 grade 2,and 12 grade 1.Results of coronary multi-slice CT imaging showed that 5 of the 97 stents implanted could not be used as the basis for imaging diagnosis and evaluation.Conclusion 64-slice spiral CT coronary angiography can be used not only to accurately ref lect the stenosis of coronary stents and lumens,but also for vascular evaluation after coronary artery bypass and stent implantation.
