天麻钩藤饮及天麻钩藤颗粒联合西药治疗肝阳上亢高血压研究

摘　　要：目前,临床使用的西药降压药大致分为六类,即α受体阻断剂、利尿剂、β受体阻断剂、钙通道阻滞剂、血管紧张素转换酶抑制剂、血管紧张素受体阻断剂。以上六类化学药物,能够使高血压病得到一定程度的控制,目前是降压的主流药物。由于个体差异,为了提高疗效,临床上中西药联合用药已被广泛应用。但随着人们生活水平的提高,人们更关注生活质量。高血压病患者除控制血压外,对生存质量的影响也成为评价降压药物疗效的一个方面。降压药物的诸多副作用,如头痛、抑郁、乏力、便秘、性功能减退,明显降低了患者的生存质量。中药天麻钩藤颗粒在降低血压的同时,能改善并提高患者的生存质量。由于高血压属于慢性病,需要长期服药,临床上在选择药物时应依据患者的实际情况,选择疗效好,安全性高的药物,中西药物联用能够起到增加疗效的同时,提高药物的安全性,但目前的研究一是研究的例数还有局限,安全性指标的观测报道不多,其作用机制还不十分清晰,为此开展系统的、大样本的疗效与安全性评价及作用机制的研究是业界急需开展的工作。对于中西药联合使用的研究,应以西医的病和中医的证相结合,从中医、西医诊断入手,借助现代科技手段,观测其疗效包括血压、心电图、中医症状的改变、生活质量的提高,并检测其血、尿、便,对其安全性如血常规、尿常规、肝功、肾功进行评价,同时开展相关组学的研究,使有效安全的药物用于临床。Presently,there are 6 types of chemical medicine on treating hypertension,that is α-receptor blocker,diuretics,β receptor blocker,calcium channel blockers,angiotensin-converting enzyme inhibitors( ACEI) and angiotensin receptor blockers( ARBs). This chemical medicine above plays a certain important role in controlling hypertension. Since individual difference and improving curative effect,it is already used universally. Except for blood pressure control,quality of life is also focused on. The side effects of antihypertensive drugs,such as bucking,constipation,headache,weaknessand sexual dysfunction,lower the quality of life. Tianma Gouteng Granules is able to lower blood pressure and improve the quality of life. The combination usage of Chinese medicine and chemical drugs can improve the curative effect and safety. Although some clinical researches have reportedthe sample is small and scattered,with few security and mechanism reports. Therefore,it is badly in need of operating a systematic,large sample researchon curative effect,safety evaluation,and mechanism. The research of drug combination should involve diseases of western medicine and syndromes of Chinese medicine,including diagnoses,modern science and technology. The observation objects for evaluating curative effect should involve blood pressure,electrocardiogram,changes of Chinese medicine syndromesand quality of life. The security reports should put blood,urine,feces,blood,liver and kidney function into consideration. All observation aboveas well ashistology-related research can provide the important reference to drug combination.
