摘  要:目的:探讨矽肺兔模型肺超声评分与胸部高分辨率CT(HRCT)的相关性。方法:选取24只新西兰兔,按随机数字表法抽取5只作为对照组,余19只作为实验组。实验组经气管注入二氧化硅粉尘(SiO2)悬浮液,建立矽肺兔模型;对照组经气管注入等量的生理盐水。造模两月后,所有兔均行肺部超声检查,留取声像图,根据B线条数、胸膜线改变、胸膜下改变进行评分;行胸部X线平片、HRCT检查,根据HRCT表现进行肺纤维化严重程度分级,分析肺超声评分与HRCT分级的相关性。结果:对照组肺部超声未探及B线数量>2根,未探及胸膜线及胸膜下改变;实验组肺部超声可探及双肺B线弥漫性增多。对照组胸部X线平片及HRCT检查显示肺纹理清晰,双侧胸膜未见增厚,胸膜腔未见积液;实验组胸部X线平片发现矽肺兔Ⅰ期3例(12.5%),矽肺兔Ⅱ期6例(21.0%),矽肺兔Ⅲ期10例(41.7%);实验组HRCT检查发现轻度肺纤维化改变3例(15.7%),中度肺纤维化改变4例(21.0%),重度肺纤维化改变12例(63.1%)。肺超声评分与HRCT分级呈正相关关系(r=0.82,P<0.05)。结论:胸肺超声可作为矽肺随访的一种补充检查。Objective:To explore the correlation between lung ultrasound score and high-resolution CT(HRCT)of chest in silicosis rabbit model.Methods:A total of twenty-four New Zealand rabbits were selected.A table of random numbers was used to choose five rabbits as the control group,and the remaining nineteen served as the experimental group.The experimental group was injected with silicon dioxide dust(SiO2)suspension through the trachea to establish a silicosis rabbit model;the control group was injected with the same amount of physiological saline through the trachea.Two months after modeling,all rabbits underwent lung ultrasonography,plain film X-ray and HRCT chest.The sonograms were retained,and scores were evaluated based on the number of B lines,pleural line changes,and subpleural changes.The severity of pulmonary fibrosis was graded based on HRCT results.The correlation between lung ultrasound score and HRCT grade was analyzed.Results:B-lines(>2),pleural line changes,or subpleural changes were not detected by lung ultrasound in control group,in contrast,increase in diffuse B-line was observed in experimental group.Plain film X-ray and HRCT examination of control group showed clear lung texture,no thickening of bilateral pleura,and no effusion in the pleural cavity;however,three silicosis rabbits(12.5%)in stage I,six rabbits(21.0%)in stage II,and ten rabbits(41.7%)in stage III were found in experimental group by plain film X-ray,and HRCT examination revealed three cases(15.7%)of mild pulmonary fibrosis,four cases(21.0%)of moderate pulmonary fibrosis,and twelve cases of severe pulmonary fibrosis(63.1%)in experimental group.Lung ultrasound score was positively correlated with HRCT grading(r=0.82,P<0.05).Conclusion:Thoracic pulmonary ultrasound can be used as a supplementary examination for follow-up of silicosis.
