摘 要:目的:探讨声脉冲辐射力弹性成像技术(ARFI)在良、恶性卵巢肿瘤中的鉴别诊断价值。方法:选取2014年1月至2018年1月广西壮族自治区人民医院收治的卵巢肿瘤患者123例,均行常规超声检查,采用ARFI技术测定病灶处剪切波速度(SWV)。根据病理结果将患者分为良性组和恶性组,恶性组再分为高分化组和低分化组,比较各组SWV值。用受试者工作特征(ROC)曲线分析SWV值对良、恶性卵巢肿瘤的诊断效能。结果:良性组SWV值为(1.97±0.36)m/s,显著低于恶性组的(2.90±0.66)m/s(P<0.05);恶性组中,高分化组SWV值为(2.52±0.57)m/s,低分化组SWV值为(3.20±0.78)m/s,均明显高于良性组(P<0.05),且低分化组SWV值明显高于高分化组(P<0.05)。以SWV值2.285 m/s为截断点,其鉴别诊断良、恶性卵巢肿瘤的ROC曲线下面积为0.854,灵敏度为81.5%,特异度为81.0%。结论:ARFI技术SWV值对良、恶性卵巢肿瘤有一定的鉴别诊断价值。Objective:To investigate the value of acoustic pulsed radiation elastography(ARFI)in the differential diagnosis of benign and malignant ovarian tumors.Methods:A total of 123 patients with ovarian lesions in our hospital from January 2014 to January 2018 were selected,and divided into benign group and malignant group according to the pathological results.The patients in malignant group were subdivided into high differentiation group and low differentiation group.All patients underwent routine ultrasound examination,and ARFI technique was used to measure the shear wave velocity(SWV)at the lesion.The SWV value was compared between groups.The diagnostic value of SWV for benign and malignant ovarian tumors was analyzed by receiver-operating characteristic(ROC)curve.Results:The mean SWV value of benign group was(1.97±0.36)m/s,which was significantly lower than that in malignant group(2.90±0.66)m/s(P<0.05).The SWV was(2.52±0.57)m/s and(3.20±0.78)m/s in high differentiation group and low differentiation group,respectively,and higher than that in benign group(P<0.05).The area under the ROC curve was 0.854 with a cutoff value of 2.285 m/s,and the sensitivity and specificity were 81.5%and 81.0%,respectively.Conclusion:ARFI technology SWV was valuable for the differential diagnosis of benign and malignant ovarian tumors.
