血小板参数在鉴别诊断免疫性血小板减少症和再生障碍性贫血中的作用及其对疗效判断的应用

摘　　要：目的探讨血小板参数在鉴别诊断免疫性血小板减少症(ITP)及再生障碍性贫血(AA)中的作用和疗效判断的临床意义。方法收集45例ITP和49例AA患者治疗前和治疗后的抗凝静脉血,选择45例健康体检者作为对照组,检测血小板参数对ITP和AA治疗前和治疗后血小板参数结果进行比较分析。结果治疗前,ITP组和AA组PLT、PCT低于对照组(P <0. 05);MPV、PDW-SD在ITP组中高于AA组(P <0. 05),AA组高于对照组(P <0. 05);P-LCR在ITP组中高于AA组和对照组(P <0. 05)。治疗后,ITP患者PLT、PCT增高(P <0. 05),P-LCR、MPV降低(P <0. 05);AA患者P-LCR、MPV均增高(P <0. 05),而PLT、PCT持续下降,上述差异均有统计学意义(P <0. 05)。结论 P-LCR、MPV、PDW-SD在ITP患者中高于AA患者,可做为两者鉴别诊断的依据。ITP患者治疗有效后P-LCR、MPV、PDW-SD显著降低,这对判断ITP治疗效果有重要的临床意义。Objective To explore the clinical significance of platelet parameters in the diagnosis and efficacy of immune thrombocytopenia( ITP) and aplastic anemia( AA). Methods A total of 45 patients with ITP and 49 patients with AA served as the observation group while another 45 healthy people undergoing physical examination as control group. Platelet parameters on every group before treatment and in observation group after treatment were compared. Results Before treatment,PLT and PCT in ITP and AA groups were significantly lower than those in control group( P < 0. 05);MPV and PDW-SD were significantly higher in ITP group than in AA group( P < 0. 05),and AA group was significantly higher than control group( P < 0. 05);P-LCR was significantly higher in the ITP group than in the AA group and the control group( P < 0. 05). After treatment,PLT and PCT in ITP patients increased significantly( P < 0. 05),P-LCR and MPV significantly decreased( P < 0. 05);P-LCR and MPV in AA patients increased significantly( P < 0. 05),while PLT and PCT continued to decline,and the differences were statistically significant( P < 0. 05). Conclusion P-LCR,MPV,and PDW-SD are significantly higher in ITP patients than in AA patients,which can be used as the basis for the differential diagnosis of the two. After effective treatment of ITP patients,P-LCR,MPV,and PDW-SD significantly reduced,which has clinical significance for judging the effect of ITP.
