摘　　要：目的观察心绞痛患者CD11b水平的变化,探讨其对不稳定型心绞痛(UAP)的预测价值。方法测定101例UAP和75例稳定型心绞痛(SAP)患者治疗前、后外周血MPA形成和CD11b表达率、白细胞(WBC)和超敏C-反应蛋白(hs-CRP)水平,采用Pearson相关性分析方法、Logistic回归和ROC曲线分析评估各指标的相关性、UAP危险因素以及危险指标对UAP的预测效能。结果UAP组MPA形成率和CD11b表达率、WBC、NLR和hs-CRP水平均显著高于SAP组(P<0.001);CD11b表达率与NLR和hs-CRP水平均呈正相关(r^2值分别为0.3917和0.2854,P<0.01);UAP有效治疗组治疗后CD11b表达率、NLR和hs-CRP水平均显著低于治疗前(P<0.001);治疗有效组各指标水平降低率显著高于无效组,上述差异均有统计学意义(P<0.01);多因素分析显示CD11b为UAP的独立危险因素(OR:4.215,95%CI:3.811~9.222;P<0.01)。ROC曲线分析显示,CD11b预测UAP的曲线下面积(AUC)为0.851(0.738~0.921),在cut-off值为8.6%时,其敏感度为83%、特异度为78%、阳性预测值为84%、阴性预测值为78%。结论CD11b表达水平升高是不稳定型心绞痛的独立风险因子,对其发生有较高的预测价值。Objective To observe the changes of CD11 b in patients with angina pectoris,and to evaluate its predictive value in unstable angina pectoris(UAP).Methods The expressions of MPA and CD11 b,the levels of white blood cell(WBC)and high sensitive C-reactive protein(hs-CRP)in 101 patients with UAP and 75 patients with stable angina pectoris(SAP)were measured before and after treatment,respectively.Pearson correlation analysis,Logistic regression analysis,and ROC curve analysis were used to evaluate the correlation of the parameters,UAP risk factors and the predictive efficacy of risk factors on UAP.Results The MPA formation rate and CD11 b expression rate,WBC,NLR,and HS-CRP levels in the UAP group were significantly higher than those in the SAP group(P<0.001);the CD11 b expression rate was positively correlated with the NLR and hs-CRP levels(r^2=0.3917,r2=0.2854,P<0.01);the CD11 b expression rate,NLR and HS-CRP levels in the UAP effective treatment group were significantly lower than before treatment(P<0.001);the reduction rates of various indicators in the effective treatment group were significantly higher than those in the ineffective group,with the above differences statistically significant(P<0.01);multivariate analysis showed that CD11 b was an independent risk factor for UAP(OR:4.215,95%CI:3.811-9.222;P<0.01).ROC curve analysis showed that the area under the curve(AUC)of CD11 b predicted UAP was 0.851(0.738-0.921),and when the cutoff value was 8.6%,the sensitivity was 83%,the specificity was 78%,the positive predictive value was 84%,and the negative predictive value was 78%.Conclusion Elevated expression of CD11 b is an independent risk factor for unstable angina pectoris,and CD11 b has a high predictive value for the occurrence of unstable angina pectoris.
