摘 要:目的探讨TCF7L2 rs7903146基因多态性与糖尿病患者血管病变及终末期肾病发生的关系。方法研究对象来源2015年6月-2017年6月本院收治的糖尿病患者240例,其中发生微血管病变的患者120例定为微血管病变组,120例未出现微血管病变的定为单纯糖尿病组,所有患者均抽血检测TCF7L2 rs7903146基因多态性,对比2组患者基因型分布特点,并分析不同基因型患者血糖控制情况,采用Logistic回归分析TCF7L2 rs7903146基因多态性与患者终末期肾脏病发生的关系。结果微血管病变组患者G/G基因分布率为55.0%,含G基因分布率为66.3%,均高于单纯糖尿病组,差异有统计学意义(P<0.05),G/T、T/T基因分别为22.5%、22.5%,均低于单纯糖尿病组,差异有统计学意义(P<0.05)。基因型为G/G的患者GLU、2h PG、HOMA-IR、Hb A1c均高于基因型为G/T和T/T的患者。Logistic回归分析结果显示,患者TCF7L2基因型为G/G、HOMA-IR、Hb A1c是糖尿病患者发生终末期肾病的独立危险因素(P<0.05)。结论TCF7L2基因型为G/G与糖尿病患者血管病变及终末期肾病发生密切相关。Objective To investigate the relationship between TCF7 L2 rs7903146 gene polymorphism and vascular lesions and end-stage renal disease in patients with diabetes mellitus.Methods A total of 280 patients with diabetes mellitus admitted to our hospital from June 2015 to June 2017 were studied.40 patients with microangiopathy were classified as microangiopathy group,240 patients without microangiopathy were classified as simple diabetes group.TCF7 L2 rs7903146 gene polymorphism was detected by blood sampling in all patients,and the gene polymorphism was compared between the two groups.The distribution of TCF7 L2 rs7903146 gene polymorphism and the incidence of end-stage renal disease were studied by Logistic regression.Results The distribution rate of GG gene was 55.0%in patients with microangiopathy and 66.3%in patients with diabetes mellitus,which were all lower than the single diabetes group,and the differences statistically significant(P<0.05).The G/T and T/T genes were 22.5%and 22.5%respectively,which were significantly lower than those in patients with diabetes mellitus,with the differences statistically significant(P<0.05).TCF7 L2 genotype G/G,HOMA-IR,Hb A1 Cwere independent risk factors for end-stage renal disease in diabetic patients(P<0.05).Conclusion TCF7 L2 genotype G/G is closely related to diabetic nephropathy and end-stage renal disease.
