急性髓细胞性白血病皮肤浸润5例

摘　　要：目的探讨急性髓细胞性白血病皮肤浸润的临床及病理学特征,提高专科医师对白血病皮肤浸润的认知。方法回顾分析5例急性髓细胞性白血病皮肤浸润患者的临床表现,皮损组织病理学检查及免疫组化结果及临床结局。结果5例患者,年龄36~65岁,皮损均为暗红、紫红色多发、泛发的丘疹、结节,浸润明显,无明显压痛。组织病理学检查表皮大致正常,表皮与真皮之间可见无细胞浸润带,真皮呈弥漫性增生的肿瘤细胞浸润;免疫组化MPO,CD45,CD68阳性率较高。结论急性髓细胞性白血病皮肤浸润临床、组织病理学均有一定的特异性,发生皮肤浸润,预后极差。Objective To explore the clinical and pathological characteristics of skin infiltration in acute myeloid leukemia,so as to improve the cognition of the specialists.Methods The clinical manifestations,histopathological examination,immunohistochemical results and clinical outcomes of 5 cases of acute myeloid leukemia were analyzed.Results Five patients,aged 36~65 years old,had dark red and purple red multiple and extensive papules and nodules,with obvious infiltration and no obvious tenderness.Histopathological examination showed that there was no cell infiltration zone between the epidermis and dermis,and the dermis was infiltrated with diffuse proliferative tumor cells;the positive rates of MPO,CD45 and CD68 were high.Conclusion The clinical and histopathological features of skin infiltration in acute myeloid leukemia are specific.The prognosis of skin infiltration is very poor.
