摘 要:目的通过联合T淋巴细胞亚群、CRP和PCT检测新生儿败血症,观察其诊断及预后的价值。方法选取本院2017年1月至2018年12月收治的疑似新生儿败血症患儿51例作为研究对象,以血培养为诊断金标准,对51例疑似患儿均进行血培养、CRP和PCT检测,观察败血症患儿检出率,对比CRP、PCT及联合检测新生儿败血症的灵敏度和特异性,分析T淋巴细胞亚群对新生儿败血症预后的评估。结果一般感染组血清中CD3^+、CD4^+及CD4^+/CD8^+水平明显高于败血症组(P<0.05);CRP+PCT联合检测的灵敏度、特异度、阳性预估值和阴性预估值明显高于单项检测。比较37例新生儿败血症患者发现T淋巴细胞亚群治疗后明显升高(P<0.05),CRP和PCT治疗后指标明显低于治疗前(P<0.05)。结论 T细胞亚群+CRP+PCT联合检测能够显著提升检测灵敏度、特异度、阳性预估值和阴性预估值,有助于新生儿败血症患儿预后。Objective To observe the value of combined detection of T lymphocyte subsets,CRP and PCT in the diagnosis of neonatal sepsis and prognosis.Methods A total of 51 children with suspected neonatal sepsis who were treated in our hospital from January 2017 to December 2018 were selected as the research objects.Using blood culture as the diagnostic gold standard,51 suspected children were tested for blood culture,CRP and PCT,and the detection rate of children with sepsis was observed.The sensitivity and specificity of CRP,PCT and combined detection of neonatal sepsis were compared,analysis of T lymphocyte subsets in assessing the prognosis of neonatal sepsisand T was analyzed.Results The serum levels of CD3^+,CD4^+ and CD4^+/CD8^+ in the general infection group were significantly higher than those in the sepsis group(P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of the combined CRP+PCT test were significantly higher than those of the single test.Comparing 37 cases of neonatal sepsis patients,the T lymphocyte subsets were significantly increased after treatment(P<0.05),and the indicators after CRP and PCT treatment were significantly lower than before treatment(P<0.05).Conclusion The combined detection of T cell subsets+CRP+PCT can significantly improve the detection sensitivity,specificity,positive predictive value and negative predictive value,and is helpful for the prognosis of children with neonatal sepsis.
