早期颅骨修补联合脑室腹腔分流术治疗脑外伤对康复效果的影响

摘　　要：目的分析早期颅骨修补术联合脑室腹腔分流术(VPS)对脑外伤患者的疗效。方法选取2016年12月至2017年2月山东省新汶矿业集团协庄矿医院收治的88例脑外伤患者,采用信封法分为观察组和对照组,各44例。两组均实施去骨瓣减压术。对照组术后3个月展开VPS手术,再过3个月展开颅骨修补术。观察组在去骨瓣减压术后展开颅骨修补术与VPS治疗。比较两组格拉斯昏迷评分(GCS)、并发症发生率及患者伤残情况。结果观察组术后并发症发生率明显低于对照组(P<0.05)。治疗后12个月,两组患者GCS评分均明显高于治疗前,且观察组GCS评分均明显高于对照组(P<0.05);治疗后12个月观察,观察组伤残情况明显优于对照组(P<0.05)。结论早期颅骨修补术联合VPS对脑外伤患者有显著治疗效果。Objective To analyze the effect of early cranioplasty combined with ventriculo-peritoneal shunt(VPS)on patients with traumatic brain injury.Methods From December 2016 to February 2017,88 patients with brain trauma admitted to xiezhuang mine hospital of shandong xinwen mining group were selected and divided into observation group and control group by envelope method,with 44 cases each.In both groups,decompression of bone flap was performed.In the control group,VPS was performed 3 months after the operation,and cranioplasty was performed 3 months after the operation.The observation group underwent cranioplasty and VPS after decompression of bone flap.GCS score,complication rate and disability were compared between the two groups.Results The incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).At 12 months after treatment,the GCS scores of the two groups were significantly higher than those before treatment,and the GCS scores of the observation group were significantly higher than the control group(P<0.05).The observation group showed that the disability of the observation group was significantly better than that of the control group at 12 months after treatment.Control group(P<0.05).Conclusion Early cranioplasty combined with VPS can exert significant therapeutic effects on patients with traumatic brain injury.
