摘 要:目的:探讨最大脂肪氧化强度跑步运动对动脉弹性改变期和斑块形成初期的延缓作用及机制。方法:采用脉搏波检测技术和彩色超声多普勒技术筛选早期动脉硬化患者80名,按照严重程度和性别随机分为动脉弹性改变对照组(AEC)、动脉弹性改变+运动干预实验组(AECE)、动脉硬化斑块形成初期对照组(APF)、动脉硬化斑块形成初期+运动干预实验组(APFE),运动干预实验组施加每周三次个人最大脂肪氧化强度跑步运动,连续干预12个月,对照组不施加任何有组织的运动计划。比较对照组与实验组肱踝脉搏波传导速度(baPWV)、踝臂指数(ABI)、颈动脉内膜-中膜厚度(IMT)、血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、肿瘤坏死因子α(TNF-α)、白介素1(IL-1)等指标。结果:运动干预后除AECE组ABI外,各实验组baPWV、ABI、IMT、TC、TG、LDL-C、TNF-α、IL-1与AEC组相比均明显下降(P<0.05),HDL-C与AEC组相比显著上升(P<0.05),AEC组各项指标和AECE组ABI与实验前没有明显改变(P>0.05),实验组baPWV、ABI、IMT、TC、TG、LDL-C、TNF-α、IL-1与AEC组相比均明显下降(P<0.05),HDL-C与AEC组相比显著上升(P<0.05)。结论:12个月最大脂肪氧化强度跑步运动能显著降低动脉弹性改变期和斑块形成初期患者的动脉硬度和动脉内膜-中膜厚度,改善血脂环境,降低氧化应激水平,减轻血管炎症状态,且动脉弹性改变期是运动干预的最佳窗口期。Objective:To investigate the effect and mechanism of maximum fat oxidation intensity running on arterial elasticity change and plaque formation.Methods:Using pulse wave detection technology and color ultrasonic doppler technology screening of early atherosclerosis in patients with 80,according to the severity and gender were randomly divided into the artery elasticity change control(AEC),arterial elasticity+exercise intervention group(AECE),early atherosclerosis plaque formation in the control group(APF),arteriosclerosis plaque formation in the early+exercise intervention group(APFE),exercise intervention group on running three times a week personal maximum fat oxidation intensity,intervention in the 12 months in a row,the movement of the control group did not exert any organization plan.Comparing the control group and experimental group brachial ankle pulse wave conduction velocity(baPWV),ankle arm index(ABI),carotid intima-in the film thickness(IMT),serum total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),tumor necrosis factor alpha(TNF-α),interleukin 1(IL-1),and other indicators.Results:After exercise intervention,except for ABI in AECE group,baPWV,ABI,IMT,TC,TG,LDL-C,TNF-αand IL-1 in all experimental groups were significantly decreased compared with AEC group(P<0.05),while HDL-C was significantly increased compared with AEC group(P<0.05).BaPWV,ABI,IMT,TC,TG,LDL-C,TNF-αand IL-1 in the experimental group were all significantly decreased compared with the AEC group(P<0.05),while HDL-C was significantly increased compared with the AEC group(P<0.05).Conclusion:The maximum fat oxidation intensity exercise at 12 months can significantly reduce the arterial stiffness and intima-media thickness of patients with arterial elasticity change period and plaque formation early stage,improve the environment of blood lipid,reduce the level of oxidative stress and vascular inflammation.
