人工全膝关节置换术对骨关节炎患者HSS评分、膝关节活动度及生存质量的影响

摘　　要：目的:探讨人工全膝关节置换术对骨关节炎患者HSS评分、膝关节活动度及生存质量的影响.方法:随机选取2016年8月-2018年8月宁夏回族自治区石嘴山市宁夏第五人民医院骨科骨关节炎患者60例,随机分为2组:一组常规抗炎治疗组(保守治疗组,30例),一组人工全膝关节置换术组(手术治疗组,30例),对2组患者的膝关节活动度、HSS评分、临床疗效、生存质量进行统计分析.结果:手术治疗组患者的WOMAC骨关节炎指数评分、VAS评分均显著低于保守治疗组(P<0.05),AKS膝、功能评分均显著高于保守治疗组(P<0.05),膝关节最大屈曲角度显著大于保守治疗组(P<0.05),HSS评分显著高于保守治疗组(P<0.05),治疗的优良率96.7%(29/30),显著高于保守治疗组76.7%(23/30)(P<0.05),S-FMMFA评分、BBS评分、RMI评分均显著高于保守治疗组(P<0.05),躯体、认知、角色、社会功能评分均显著高于保守治疗组(P<0.05).结论:人工全膝关节置换术较常规抗炎治疗更能有效提升骨关节炎患者HSS评分、膝关节活动度及生存质量.Objective:To investigate the effects of total knee arthroplasty on HSS score,knee joint mobility and quality of life of pa-tients with osteoarthritis.Methods:60 cases of patients with orthopaedic osteoarthritis in Ningxia Fifth People's Hospital of Shizuishan Cit-y,Ningxia Hui Autonomous Region from August 2016 to August 2018 were randomly selected and divided into two groups:one group were treated with conventional anti-inflammatory therapy(conservative treatment group,30 cases),the other group were treated with total knee arthroplasty(operation treatment group,30 cases).The knee joint activity,HSS scores and clinical curative effect and quality of life of the two groups were statistically analyzed.Results:The WOMAC Osteoarthritis Index score and VAS score of the surgical group were significantly lower(P<0.05),the AKS knee and function score were significantly higher(P<0.05),the maximum flexion angle of knee joint was significantly higher(P<0.05),the HSS score was significantly higher(P<0.05),the excellent and good rate of treat-ment 96.7%(29/30)was significantly higher than the conservative treatment group 76.7%(23/30)(P<0.05),the scores of S-FMMFA,BBS and RMI were significantly higher(P<0.05),the scores of somatic,cognitive,role and social function were significant-ly higher than the conservative treatment group(P<0.05).Conclusion:Total knee arthroplasty was more effective than conventional anti-inflammatory therapy in improving HSS score,knee joint mobility and quality of life of patients with osteoarthritis.
