摘  要:目的比较血结核抗体、痰TBDNA试验与T细胞斑点试验(T-SPOT.TB)检测方法在糖尿病合并菌阴肺结核诊断中的临床价值。方法选取2017年1月至12月上海市奉贤区古华医院收治的疑似糖尿病合并菌阴肺结核患者48例,最后诊断糖尿病合并菌阴肺结核患者35例为观察组,其余13例为对照组,均予以血结核抗体、痰TBDNA试验与T-SPOT.TB检测,统计并比较3种检测方法的灵敏度、特异度、符合率。结果T-SPOT.TB对结核感染的灵敏度及符合率明显高于血结核抗体、痰TBDNA试验(P<0.05)。结论T-SPOT.TB在糖尿病合并菌阴肺结核诊断中具有高灵敏度,高特异度,高符合率等特点,在辅助诊断方面具有重要价值。Objective To compare the clinical value of blood tuberculosis antibody,TBDNA test and T cell spot test(T-SPOT.TB)in the diagnosis of diabetes mellitus with bacterial negative tuberculosis,to observe the T-cell spot test(T-SPOT.TB).Methods A total of 48 patients with suspected diabetes mellitus complicated with bacterial negative tuberculosis admitted to Fengxian District Guhua Hospital in Shanghai from January to December 2017 were selected.Finally,35 patients with diabetes mellitus complicated with bacterial negative tuberculosis were observed in the observation group,and the remaining 13 patients were in the control group.All of them had been tested for blood tuberculosis antibody,TBDNA test and T-SPOT.TB.The sensitivity,specificity,and coincidence rate of the three detection methods were compared.Results The sensitivity and coincidence rate of T-SPOT.TB to tuberculosis infection were higher than the blood tuberculosis antibody and TBDNA test(P<0.05).Conclusion The T-SPOT.TB has the characteristics of highly sensitive,highly specific,and highly consistent in the diagnosis of diabetes mellitus with bacterial negative tuberculosis with a great diagnostic value.
